静脉局部麻醉在肢体手术中的应用

摘　　要：目的观察0.5%利多卡因的静脉局部麻醉效果。方法选择踝和腕关节以下手术部位的患者243列,其中手部184列,足踝部59列,患者(ASAⅠ～Ⅱ级),向穿刺部位主注入容易手部0.5mL(/mg kg)总量少于40mL(用生理盐水稀释),足部1mL(/mg kg)总量<70mL,进行静脉局部麻醉,每5min一次,测量感觉和运动神经阻滞时间;手术结束,松开止血带后,每5min一次测量感觉和运动神经恢复时间;记录术后镇痛时间。结果静脉局部麻醉有起效快,镇痛完善无需强化。结论对机体生理功能干扰小,并发症少,操作简单,费用低等优点,非常适合基层医院短小骨科手术。Objective To observe the 0.5% lidocaine vein local anesthesia effect.Method Choose the ankle and wrist surgery parts of the 243 patients listed,including 184 fathers hand,his ankle department 59 column,patients（ASAⅠ～Ⅱlevel）,to puncture the Lord into the hand to 0.5mL/（mg kg） total amount less than 40mL（with saline water dilute）,foot 1mL/（mg kg） total amount is less than 70mL,an iv local anesthesia,every 5min once,measuring sensory and motor nerve block of time;Operation over to loosen a tourniquet,every 5 min a measurement feeling and motor recovery time;Postoperative analgesia record time.Results Intravenous local anesthesia with rapid onset of analgesia perfect without strengthening.Conclusion The physiological functions of the body interference,fewer complications,simple operation,and low cost,very short and orthopedic surgery for primary hospital.
