中药灌肠剂治疗糖尿病肾病临床研究

摘　　要：目的:观察水蛭、大黄、丹参、黄芪等中药煎剂合α酮酸治疗2型糖尿病导致慢性肾功能衰竭的临床疗效。方法:46例住院治疗的2型糖尿病肾病进入到慢性肾功能衰竭期,根据个人意愿分成对照组(n=19)和治疗组(n=27),分别接受降糖、降压、改善循环、静滴肾必需氨基酸、部分加用α酮酸等治疗,中药灌肠组在普通治疗基础上给予中药汤剂口服加直肠透析治疗;在治疗前、后分别采血,测定血糖及相关指数、血脂、肾功能等。结果:治疗组年龄高于普通治疗组(P<0.01),血糖、胰岛素浓度、血脂各项组间无差别;治疗前,中药灌肠组的血肌酐和血尿酸均显著高于普通治疗组,通过治疗两组的肾功能均有改善,中药灌肠组有效率显著高于对照组(但P>0.05),中药灌肠组的平均血肌酐显著下降(P<0.01);结论:中药汤剂在治疗2型糖尿病的慢性肾功能衰竭期中,可以进一步提高治疗效果。Objective ： To observe the clinical curative effect of Decoction of leech, pieplant, salvia miltiorrhiza, and Astragalus mongholicus combined with α-keto-acid on chronic renal failure resulted from type 2 diabetes. Methods： 46 cases hospitalized by chronic renal failure resulted from type 2 diabetes were divided into two groups according to personal will. All the cases were treated with the methods of stepping-down-blood-pressure, cutting- down-blood-sugar, improving blood circulation, drop-infusing kidney-essential amino acid intervenously, and giving to ct-keto-acid partly. Besides above those the TCM decoction was given to treatment group as well as rectum dislysis with Chinese medicine. Before and after treatment blood-sugar, blood-fat and renal function were determined, and analyzed statistically. Results： The age in treatment group was higher than that in control group （P = 0.07）, and there was no difference between the groups on blood sugar, insulin concentration and blood-fat. Before treatment serum creatinine and blood uric acid in treatment group were higher than those in control group. After treatment renal functions in the two groups were improved, and the effective rate in treatment group was better than that in control group （ P 〈 0.05 ）. And the average serum creatinine decreased markedly in treatment group （ P 〈 0.01 ）. Conclusion ： The decoction in our department can improve the clinical effect on chronic renal failure resulted from type 2 diabetes.
