摘　　要：目的:探讨乙肝表面抗原(HBsAg)携带孕妇胎盘Toll样受体(TLRs)的表达与新生儿外周血乙肝表面标志物(HBV-Ms)的关系。方法:收集2010年6月至12月在暨南大学附属第一医院产检并分娩的HBsAg携带孕妇53例,正常孕妇10例。ELISA法检测血清HBV-Ms;荧光定量PCR法检测HBV-DNA;Envision免疫组化法检测胎盘TLR3、TLR4、TLR7表达。结果:(1)HBsAg携带孕妇胎盘中TLR3、TLR7的表达显著高于正常胎盘组织(P<0.05),各组间TLR4的表达无统计学差异。(2)新生儿HBsAb阳性者胎盘TLR3的表达显著高于阴性者及正常对照组(P<0.05)。结论:胎盘TLR3、TLR7可能参与HBV垂直传播的免疫调控;胎盘TLR3的高表达与新生儿HBsAb阳性密切相关,是抗HBV垂直传播的保护性因素。Objective：To explore the relationship between the expressions of placental toll-like receptors（TLRs） in the hepatitis B surface antigen（HBsAg） carriers and Hepatitis B surface markers（HBV-Ms）in peripheral blood of their fetuses.Methods：53 HBsAg carriers and 10 normal pregnant women recruited in this study were regularly performed perinatal care in the First Affiliated Hospital of Jinan University from June to Dec.2010.Serum HBV-Ms,hepatitis B virus-deoxyribose nucleic acid（HBV-DNA）and the expressions of placental TLR3,TLR4 and TLR7 were respectively detected by ELISA,RT-PCR and Envision immunohistochemistry.Results：（1）The expressions of placental TLR3 and TLR7 in HBsAg carriers were stronger than those of normal gravidas.The expression of placental TLR4 in each group was not statistically different.（2） Expression of placental TLR3 in HBsAb-positive newborns was significantly stronger than those of HBsAb-negative and control newborns.Conclusion：Placental TLR3 and TLR7,perhaps,join in the immunoregulation of HBV vertical transmission.Stronger expression of placental TLR3 was closely linked to HBsAb positive in newborns,which indicates that TLR3 may be a protective factor of HBV vertical transmission.
