摘　　要：目的探讨以指动脉背侧皮支为蒂的岛状皮瓣修复同指指背皮肤缺损的临床效果。方法2008年1月至2010年2月，对12例不同节段指背皮肤软组织缺损，采用以近创面指动脉背侧皮支为蒂的岛状皮瓣修复同指指背缺损，其中顺行推进皮瓣7例，逆行岛状皮瓣5例。结果术后皮瓣均顺利存活，供、受区切口均I期愈合。10例获得随访，时间为6个月至2年；2例失访。皮瓣质地与周围组织接近，外形无臃肿；根据手指关节总活动度（TAM）系统评定：优8例，良1例，可1例。结论采用指动脉背侧皮支为蒂的岛状皮瓣是一种修复同指指背皮肤缺损的简单、有效的治疗方法。Objective To investigate the therapeutic effect of transferring island flap pedicled with digital artery dorsal cutaneous branch for dorsal skin defect at the same finger. Methods From January 2008 to February 2010, 12 cases of skin defect at the dorsum of the finger were treated with an island flap from the same finger pedicled with digital artery dorsal cutaneous branch. Among them 7 cases were treated with antegrade advancement flap, while 5 cases were treated with reverse island flap. Results All flaps survived completely. Both the donor and recipient sites achieved primary wound healing. Ten cases were follow-up for 6 to 24 months after the operation. The flaps had good texture and color match. Two-peint discrimination of the advancement flaps was 8 to I0 ram, while 2-point discrimination of the reverse flaps was 10 to 12 mm. According to the criteria for funcl^onal assessment by total action marion （TAM）, 8 cases were regarded as having excellent results, 1 case good result and 1 case fair result. Conclusion Island flap pedicled with digital artery dorsal cutaneous branch is an ideal procedure for treating dorsal skin defect at the same finger.
