摘 要:目的观察植入式BAHA(Bone-anchored hearing aids,骨锚式助听器)在双侧先天性中外耳畸形患者的听力改善效果,探索BAHA规范植入技术。方法在一侧耳后区颞骨上植入BAHA钛质植入体,成人一期完成,小儿分二期完成。比较3例双侧先天性中外耳畸形患者,佩戴植入式BAHA前后的听力情况,包括日常交流、纯音(声场)测听或条件反射测听。结果 3例患者均手术顺利,术后恢复好。植入式BAHA佩戴后,患者及/或家属反映日常交流明显改善,纯音(声场)测听或条件反射测听0.25k~4k平均气导阈改善39.1dB。结论植入式BAHA对双侧先天性中外耳畸形患者是一种有效的听力解决方案,规范的植入技术是手术成功、减少并发症的保证。Objective To report hearing outcomes of BAHA (Bone-anchored hearing aids) implant in patients with bilateral congenital malformation of middle and outer ears, and to discuss standard surgical techniques for BAHA implantation. Method Unilateral BAHA titanium implants were implanted in the temporal bone posterior to the auricle. The proc;edure was completed in one stage in adults and two stages in children. Pre- and post-implant hearing results in 3 cases were compared, including daily communication, pure tone (sound field) audiometry in adults or conditioned reflex audiometry in child. Results The data showed that the 3 patients improved significantly in daily communication and in hearing thresholds after BAHA implant . Pure tone (sound field) audiometry or conditioned reflex audiometry thresholds over 0.25-4 kHz was improved with an average of 39.1 dB. Conclution BAHA implant is an effective hearing solution for patients with bilateral congenital middle and outer ear malformation. The standard surgical techniques of BAHA implant shonld ensure success of operation and minimal complications.
