摘  要:目的:检测IgA肾病肾小球内旷平滑肌肌动蛋白(α-SMA)及波形蛋白等细胞骨架蛋白表达,探讨IgA肾病系膜细胞肌成纤维细胞表型转化与病理分级、肾功能关系。方法:对63例肾穿刺病理诊断为IgA肾病患者,回顾其临床资料及病理改变,通过免疫组织化学显色检测α-SMA及波形蛋白在。肾小球的表达。参考Cockcrof-Gault公式计算肾小球滤过率(GFR)。结果:α-SMA、波形蛋白在IgA肾病标本中均100%阳性表达,α-SMA及波形蛋白在肾小球内的表达呈正相关关系。α-SMA、波形蛋白表达与WHO分级、Katafuchi氏。肾病积分及GFR均呈正相关关系。在与GFR相关性方面,rsMA高于波形蛋白。结论:α-SMA可作为评估IgA肾病患者肾病变严重程度的分子生物学指标。Objective: To detect the expression of cytoskeletal proteins alpha smooth muscle actin (a-SMA) and vimentin within glomeruli in the IgA nephropathy, and to investigate the correlation with mesangial cells myofibroblast phenotype transition, patho- logical grading and kidney function. Methods. Sixty-three renal biopsies were collected from the patients with IgA nephropathy, and their the clinical data and pathological changes were reviewed. Elivision immunohistochemical staining method was used for detecting the expression of a SMA and vimentin within glomeruli. Referring to the Cockcroft-Gault formula, the glomerular filtration rate (GFR) was calculated. SPSS19. 0 software was applied for statistical analysis. Results. The expression of a-SMA and vim- entin in specimens of 63 patients with IgA nephropathy was 100% positive The expression of a-SMA was positively correlated with vimentin in glomeruli. Meanwhile the a-SMA and vimentin protein expression was found positively correlated with WHO classi- fication, Katafuchi serni-quantitative pathological scoring system and GFtL The correlation between GFR and a-SMA was higher than vimentin. Conclusion: a-SMA can be taken as a molecular biological indicator to evaluate the severity of IgA nephropathy.
