摘  要:目的:为临床带血供的第1骨间背侧肌远端肌支神经肌蒂转移修复拇对掌功能手术提供供区肌肉神经、血管相关解剖学依据,评估切取该肌远端肌支对供区的影响。方法:手部固定标本,采用显微解剖方法,观察第1骨间背侧肌形态和神经支配情况。通过再改良Sihler’s染色法对第1骨间背侧肌进行肌内神经染色。结果:第1骨间背侧肌神经来源于尺神经深支,在进入第1骨问背侧肌前分成2支。其中近端分支主要分布于肌腹的上2/3,远端分支主要分布在肌腹的下1/3。结论;切取第1骨间背侧肌远端肌支神经肌蒂转位修复拇对掌肌的功能是可行的,其对供区功能的影响较小。Objective. To provide anatomical basis for neuromuscular pedicle of the first dorsal interosseous muscle branch with blood supply transposition to renovate opponens pollicis, and to study the blood supply and nerve innervation of it and value the affection of cutting the distal nerve branch of the first dorsal interosseous muscle. Methods: Ten formalin fixed hands were dissected. The morphology and the nerve innervation were investigated. The modified Sihler's neural staining technique was used to observe the distribution of the first dorsal interosseous muscle. Results: The nerves of the first dorsal interosseous muscule was from the profundus branch of the unlar nerve. The nerves diversed two branches and each of them entered into the upper or lower 1/3 segment of the muscle. The proximal branch distributed in the upper 2/3 of the muscle, and the distal branch distributed in the lower 1/3. Conclusion: It is feasible to renovate the opponens pollicis by transferring neuromuscular pedicle of the distal branch of the nerves of the first dorsal interosseous muscle. The damage of cutting the distal branch of the nerves of the first dorsal interosseous muscle is limited.
