重建钢板内固定治疗肩胛骨骨折的临床疗效评价

摘　　要：目的探讨重建钢板内固定治疗肩胛骨骨折的临床治疗效果。方法 2007年1月～2009年12月笔者收治的42例肩胛骨骨折患者为研究对象,随机分成对照组和观察组各21例,对照组给予T型锁定钛板内固定治疗,观察组给以重建钢板内固定治疗,随访观察2年,用Hardegger标准对治疗效果进行评价,并观察并发症发生情况。结果观察组21例肩胛骨骨折患治疗疗效为优12例,良6例,中2例,差1例,优良率为85.71%,发生创伤性肩关节炎1例,畸形愈合1例,并发症发生率为8.52%;对照组21例肩胛骨骨折患治疗疗效为优10例,良7例,中3例,差1例,优良率为80.95%,发生创伤性肩关节炎2例,畸形愈合1例,并发症发生率为14.26%;两组比较差异有统计学意义(P<0.05)。结论重建钢板内固定能维持肩胛骨的正常解剖结构和功能,术后早期进行功能锻炼,可减少和预防并发症,治疗效果良好,并发症发生率较低,是一种有效治疗肩胛骨骨折的手术方法。Objective To explore and discuss the clinical efficacy of reconstruction plate fixation for the treatment of scapular fracture.Methods We chose 42 cases of scapular fracture patients as study object in our hospital from Jan.2007 to Dec.2009,and divided into two groups,the observation group and the control group,each group with 21 patients.The control group was given T locked titanium plate fixation treatment,while the observation was given reconstruction plate fixation treatment.We evaluated the clinical efficacy of them under Hardegger standard and observed whether complications happened in these two groups.Results The efficacy rate of the observation group was 85.71%,12 cases with excellent efficacy,6 cases with good efficacy,2 cases with efficacy and 1 case invalid.While he efficacy rate of the control group was 80.95%,10 cases with excellent efficacy,7 cases with good efficacy,3cases with efficacy and 1 case invalid.The rate of complications was 8.52% in the observation group with 1 case of traumatic shoulder and 1 case of mal-union.While the rate of complications was 14.26% in the control group with 2 cases of traumatic shoulder and 1 case of mal-union.The difference in these two groups was of statistical significance （P 0.05）.Conclusion The reconstruction plate fixation treatment can maintain the function and normal anatomic structure of scapular.Functional exercise in the early stage after the treatment can not only prevent and reduce complications,but also achieve a good clinical effect.Therefore it is a good method in the treatment of scapular fracture.
