血浆Hcy和hsCRP与心绞痛患者冠脉病变的关系

摘　　要：目的探讨不稳定性心绞痛患者血浆同型半胱氨酸(Hcy)和高敏C反应蛋白(hsCRP)水平与冠状动脉病变程度的关系。方法不稳定型心绞痛(UAP)组52例,另设性别、年龄相匹配经冠脉造影排除冠心病的患者作为对照组(35例)。所有研究对象均于入院后空腹采血后分离血浆,检测血浆Hcy和hsCRP,经冠状动脉造影检查后采用Gemini法评估冠脉病变狭窄程度。结果不稳定性心绞痛组与对照组患者Hcy分别为(14.5±3.9)μmol/L,(7.95±2.8)μmol/L(P<0.05);hsCRP分别为(8.7±1.3)μmol/L,(2.7±1.2)μmol/L(P<0.05),Hcy在不稳定性心绞痛患者中随着冠脉病变程度的加重而升高,而hsCRP水平与冠脉病变程度无相关性。结论随着冠脉病变程度的增加,血浆Hcy水平增高,而hsCRP不与冠脉严重程度成正相关,通过测定Hcy可以反映冠脉狭窄的严重程度,而测定CRP可以反映冠脉不稳定斑块的活动程度。Objective To study the relation between the plasma Homocysteine （Hcy）, High sensitivity C reactive protein （hsCRP） and degree of coronary artery lesions in patients with unstable angina pectoris. Methods Total 52 patients with unsta- ble angina pectoris in our hospital were selected as unstable angina pectoris（UAP） group, and 35 sex and age-matched patients who were eliminated of coronary heart disease by coronary angiography were as the control group;All the objects was taken of fast- ing blood samples and their plasma were separated, and the Hcy and hsCRP were detected. Then the degree of coronary artery ste- nosis was evaluated by Gemini score after the coronary angiography. Results The Hcy of unstable angina pectoris group and con- trol group were （ 14.5 ＋ 3.9 ） μmol/L, （ 7.95 -＋ 2.8 ） μmoL/L （ P 〈 0.05 ）, the hsCRP were （ 8, 7 ＋ 1.3 ） μmoL/L, （ 2.7 ＋ 1.2 ） μmol/L（ P 〈 0.05 ） ,the Hcy increased with the degree aggravation of coronary artery lesions ,while the hsCRP had no correlation to the degree of coronary artery lesions. Conclusion With the aggravation of coronary artery lesions,plasma Hcy levels elevated obviously ; the level of hsCRP correlated positively with coronary severity, Hcy can reflect the severity of coronary stenosis, and the determination of CRP can reflect the level of activitv of coronary unstable nlaaues.
