摘　　要：目的探讨改良手术方法治疗双侧臀肌筋膜挛缩症的疗效及临床价值。方法 2005年6月-2011年6月采用挛缩带"Z"形切开松解延长手术治疗31例双侧臀肌筋膜挛缩症患者,并随访6～24个月。结果伤口Ⅰ期愈合29例,延期愈合2例;按黄耀添评价标准:步态、并膝下蹲、交腿试验、对运动与体力劳动影响,4项正常者为优,26例。良:步态正常,并膝不能完全下蹲(中立位屈髋120°～130°),和/或交腿试验完成稍差(髋内收10°～20°),对运动与体力劳动基本无影响4例;可:轻度外"八"字步态,并膝下蹲受限(中立位屈髋90°～120°),和/或交腿试验完成差(髋内收0°～10°),对运动与体力劳动有一定影响1例;差:手术无效。本组无疗效差病例。手术优良率96.7%。结论挛缩带"Z"形切开松解延长术是治疗臀肌筋膜挛缩症的最佳方法之一。臀部外形恢复满意,确保髋关节术后稳定。Objective To explore the clinical efficacy and value of Z-plasty in the treatment of bilateral gluteal muscle contrac- ture（GMC）. Methods From June 2005 to June 2011,31 patients with bilateral GMC were treated by using Z-plasty. The follow up was 6 to 24 months. Results The operative incisions in 29 cases patients were primary healing,2 cases were delayed healing. According to Huang＇ s criteria,26 cases were outstanding（ scores including gait,knee squats, cross legs test, influences on move- ment and physical labor;outstanding means all 4 items were favorable）. 1 case patient was passably. None of the patients were poor effects. The curative rate was 96.7% ;4 cases were good;1 case was fair. The rate of excellent and good cases was 96.7%. Conclusion Z-plasty is one of the best approaches for the gluteal muscle eontracture. The outlines of buttocks recovered satisfac- torily, and hip joints are stable.
