输卵管妊娠治疗后再次妊娠的结局分析

摘　　要：目的探讨输卵管妊娠治疗后再次妊娠的类型及宫内妊娠率。方法回顾性分析我院2006年10月～2009年10月338例输卵管妊娠的患者资料,以采用药物保守治疗、输卵管病灶清除术(保守性手术)及输卵管切除术,分别比较患者治疗后再次妊娠的结局及宫内妊娠的情况。结果药物治疗组与手术治疗组输卵管通畅率分别为65.1%,91.2%,差异有显著性。宫内妊娠率分别是:药物治疗组63.16%,保守性手术组85.50%输卵管切除组68.75%,再次异位妊娠率药物治疗组与手术组有显著性差异。结论输卵管妊娠的治疗应根据具体的情况而定,输卵管切除术适用于无生育要求或输卵管妊娠破裂导致输卵管难以修复者,年轻有生育要求者慎用;药物治疗创伤小,费用相对较低,但治疗随访周期长;保守性手术治疗输卵管通畅率高,术后妊娠率也高。Objective To investigate the types and rate of re-pregnancy after treatment of tubal pregnancy.Methods Clinical data of 338 cases of tubal pregnancy in our hospital from October 2006 to October 2009 were retrospectively analyzed.All the cases had been treated with conservative medication,fallopian tube debridement（conservative） and fallectomy.The results of re-pregnancy after treatment of tubal pregnancy and intrauterine pregnancy were compared.Results The fallopian tube patency rate of the medication group and the conservative surgical group was respectively 65.1% and 91.2%.The difference was apparent.As for the rate of uterus pregnancy,the conservative medication group was 63.16% in contrast with 85.50% of the conservative surgical group and 68.75% of the fallectomy group.There was a significant difference between the medication group and the surgical group in terms of the rate of re-pregnancy.Conclusion The treatment of the tubal pregnancy should vary with the situation.Fallectomy is suitable for those who have no bearing demand or who suffer from tubal rupture with pregnancy and failure in the tube repair.Medication treatment is typical of low harm,low cost,but the follow-up is relatively long.Conservative surgical treatment is recommended for its high rates of fallopian tube patency and post-operative pregnancy.
