滋阴解毒祛瘀法对60例SLE患者生存质量的影响

摘　　要：目的系统评价中医滋阴解毒祛瘀法对系统性红斑狼疮(SLE)生存质量的影响。方法:采用随机、对照、双盲的临床研究设计方案,收集60例肝肾阴虚、热毒血瘀证轻中度活动SLE患者,按随机法分为治疗组和对照组,2组均给予相同西医基础治疗(糖皮质激素+羟氯喹或环磷酰胺),其中治疗组采用滋阴解毒祛瘀方,并根据临床症状随症加减;对照组采用中药安慰剂。结果治疗24周后,随着病情缓解稳定,治疗组、对照组2组患者在躯体功能(PF)、躯体职能(RP)、躯体疼痛(BP)、总体健康(GH)、生命活力(VT)、社交功能(SF)、情感职能(RE)、精神健康(MH)、SF-36总分等方面得分均有所升高。用药24周后,在躯体职能(RP)方面,治疗组(62.50±42.22)分,对照组(37.50±44.30)分,2组组间比较差异有统计学意义(P<0.05);用药24周后,SF-36总积分治疗组(479.21±156.47)分,对照组(415.23±155.61)分,2组组间比较差异有统计学意义(P<0.05)。结论滋阴解毒祛瘀方能提高SLE患者生存质量。OBJECTIVE To systematically evaluate effects of nourishing yin,detoxication and removing blood stasis methods on survival quality of SLE patients.METHODS Random control and double blind clinical research design was applied in 60cases of liver and kidney yin deficiency,heat toxicity and blood stasis of gentle and moderate movable SLE patients;they were randomly divided into treatment group and control group with both basic western medicine treatment（glucocorticoid＋ hydroxychloroquine/cyclophosphane）,one of which was also treated by the method of nourishing yin,detoxication and removing blood stasis and the formula was modified according to the symptoms.The control group was also treated by placebo.RESULTS After 24wks treatment,with the gradually relieved symptoms,scores of PF,RP,BP,GH,VT SF,RE,MH SF-36 in both treatment group and control improved.In RP,the treatment group scored（62.50±42.22）,control group scored（37.50±44.30）,which showed significant differences（P0.05）;in SF-36,treatment group scored（479.21±156.47）and control group scored（415.23±155.61）,which showed significant differences（P0.05）.CONCLUSION Method of nourishing yin,detoxication and removing blood stasis can improve SLE survival quality.
