中医药综合方案对脑卒中社区康复患者生存质量及心理状态的影响

摘　　要：目的观察中医药综合方案对脑卒中社区康复患者的生存质量及心理状态的影响。方法将327例患者随机分为观察组176例和对照组151例。两组患者均给予常规社区基础康复治疗,观察组在此基础上加用中医药综合康复方案,包括中药(中成药)辨证治疗、针刺治疗。分别于治疗前、治疗后3个月和治疗后6个月采用功能综合评定量表(FCA)对两组患者进行生存质量评定,采用汉密尔顿焦虑自评量表(SAS)和抑郁自评量表(SDS)进行心理状态的评价。结果治疗后3个月、治疗后6个月,两组FCA评分与治疗前比较差异均有统计学意义(P<0.05)。观察组治疗后3个月和6个月FCA评分比较差异有统计学意义(P<0.05)。两组治疗后3个月及治疗后6个月组间比较差异均有统计学意义(P<0.05)。两组治疗后3个月及治疗后6个月,患者SAS、SDS评分均较治疗前明显改善(P<0.05),且观察组优于对照组(P<0.05)。两组治疗后3个月和治疗后6个月SAS、SDS评分比较差异均有统计学意义(P<0.05)。结论中医药综合方案能够提高脑卒中社区康复患者生存质量,改善患者焦虑和抑郁状态,并且优于单纯康复治疗。Objective To observe the effect of combination therapy of Chinese medicine on the quality of life（QOL） and mental state in community-based rehabilitation for stroke patients.Methods Totally 327 patients were randomized into observation group of 176 cases and control group of 151 cases.Both groups were given community-based rehabilitation.The observation group was added with the combination therapy of Chinese medicine such as Chinese patent medicine and acupuncture.The improvement of QOL was assessed by functional comprehensive assessment（FCA） and the mental state was assessed by Hamilton Self-Rating Anxiety Scale（SAS） and Self-Rating Depression Scale（SDS） before treatment and as compared with 3 or 6 months after treatment.Results The FCA scores were significantly increased in both groups 3 and 6 months after treatment（P〈0.05）.After 6 months,the FCA score was significantly increased in the observation group,with statistical difference as compared with that after 3 months（P〈0.05）.Difference between groups after 3 and 6 months was significant（P〈0.05）.The scores of SAS and SDS were significantly improved in both groups 3 and 6 months after treatment（P〈0.05）,especially in the observation group（P〈0.05）.The difference in SAS and SDS score between groups after 3 and 6 months was also significant（P〈0.05）.Conclusion The combination therapy of Chinese medicine could improve QOL and mental state of the community-based rehabilitation of stroke patients.Its efficacy is superior to that of simple rehabilitation therapy.
