低浓度利多卡因骶管麻醉在老年膀胱镜检中的应用

摘　　要：目的探讨低浓度利多卡骶管麻醉对减轻老年病人膀胱镜检查痛苦的效果及安全性。方法将60例老年患者随机分为低浓度利多卡因骶管麻醉组与常规局部黏膜表面麻醉组行膀胱镜检,每组均为30例。观察麻醉和镜检过程中的各项指标,比较其优缺点。结果两组对照,骶管麻醉组(骶管组)在减轻患者痛苦、尿道松驰、医生操作满意度以及对患者生命体征影响等方面优于粘膜表面麻组(表麻组)。两组镇痛效果比较,镜检过程中无痛和轻微疼痛病例,VAS评分0～3分骶管组为28例(93.33%),明显高于表麻组11例(36.67%),差异有统计学意义(P<0.05)。结论低浓度利多卡因骶管麻醉下行膀胱镜检效果良好,极大地提高了麻醉质量,安全可靠,并发症少,对老年病人尤为适用。Objective To explore a safe and effective method to alleviate the pain of senior patients from cystoscopy.Methods 60 senior patient with cystocopy were randomly divided into low-concentrate lidocaine sacral canal anesthesia group and routine local surface anesthesia group,with 30 patients each group.The indicator of the anesthesia process,their advantages and disadvantages were observed.Results The low canal group was remarkably better than the surface group in aspects of pain alleviation of patients,urethra relaxation,doctor＇s satisfaction on operations and impact on vital signs of patients.Comparing the pain alleviation effect of the two groups,there were 28 cases（93.33%） of no pain or minor pain during the cystoscopy process and VAS score 0-3 in the low canal group,which was significantly more than the 11 cases（36.67%） in the surface group.There was statistical significance in the difference（P〈0.05）.Conclusion The effect of cystocopy using low-concentrate lidocaine sacral canal anesthesia is fair.It significantly improves the quality of anesthesia,and is safe and reliable with less complication,which is especially suitable for the senior patients.
