尿道扩张防治经尿道前列腺电切汽化术后尿道狭窄的研究

摘　　要：目的：探讨近期尿道扩张法对于良眭前列腺增生经TuRP术后发生尿道狭窄的防治作用。方法：回顾陛分析苏州市市立医院北区泌尿外科于2008年8月-2011年3月采用尿道扩张法治疗36例良性前列腺增生经TuRP术后发生尿道狭窄的临床资料。结果：36例病例中，出院4周内发生尿道狭窄1例，占2．8％；4-8周发生13例，占36．1％，8-12周发生15例，占41．7％，12—16周出现者7例，占19．4％。数据表示，TuRP术后出现尿道狭窄集中发生于治疗后4--12周。36例患者经尿道扩张的平均次数为8次。经治疗后，所有病例均尿线增粗，排尿通畅，治愈率达100％。36例患者经尿道扩张治疗后，无一例失败而采用其他方法，随访3月无尿道狭窄复发，随访6个月2例尿道狭窄复发，随访12个月1例尿道狭窄复发。结论：早期诊断，术后密切随访，积极治疗，采取近期尿道扩张术可以有效防治远期TURP术后发生的尿道狭窄，极大提高患者生活质量。Objective ：To investigate the clinical benefits of urethral dilatation on treating and preventing from the complication of urethral stricture post transurethral resection of prostate.Methods ：A retrospective clinical analysis was performed on the clinical documents of 36 patients with benign prostatic hyperplasia undergone TUIRP in Suzhou Municipal Hospital during August 2008 to March 2011.Results ：Among 36 subjects, one patient suffered urethral stricture within 4 weeks post discharge with a ratio of 2.8%, 13 patients suffered urethral stricture 4-8 weeks post discharge with a ratio of 36.1%, 15 patients suffered urethral stricture 8-12 weeks post discharge with a ratio of 41.7%, and 7 patients suffered urethral stricture 12-16 weeks post discharge with a ratio of 19.4%.The above data shown that urethral stricture occurred mainly at 4-12 weeks post the treatment of transurethral resection of prostate.Expanding urethral was carried out at a mean frequency of 8 times.After treatment, all patients satisfied with urination with a cure rate of 100%, and recrudescence of urethral stricture was observed at neither case within 3 months of following-up, at 2 cases within 6 months of following-up, and at 1 case within 12 months of following- up.Conclusion ：Early diagnosis, close postoperative following-up, and taking urinary dilatation play an important role on the treatment and prevention in the urethral stricture post TURP, which will greatly improve patients＇ quality of life.
