沙美特罗替卡松联合孟鲁司特对哮喘患儿免疫功能的影响及疗效分析

摘　　要：目的探讨沙美特罗替卡松联合孟鲁司特对哮喘患儿肺功能、免疫功能的影响及临床疗效。方法 100例支气管哮喘患儿随机分为治疗Ⅰ组和治疗Ⅱ组。治疗Ⅰ组每日给予沙美特罗替卡松2次。治疗Ⅱ组在给予沙美特罗替卡松的基础上,每日睡前口服孟鲁司特5 mg。疗程均为3个月。治疗前后分别测定患儿的肺功能、外周血IL-4、INF-γ、IgE和嗜酸性粒细胞(EOS)的水平及哮喘控制测试评分。结果两组治疗后肺功能均较治疗前明显好转,ACT评分均较治疗前明显增高。且治疗Ⅱ组较治疗Ⅰ组治疗后也有显著肺功能改善和ACT评分增高。两组治疗后肺功能IL-4、IgE、EOS水平和IL-4/IFN-γ均较治疗前明显降低,而INF-γ较治疗前明显升高。治疗Ⅱ组较治疗Ⅰ组治疗后也有显著的IL-4、IgE、EOS水平和IL-4/IFN-γ的降低及INF-γ的升高。结论沙美特罗替卡松联合孟鲁司特较单用沙美特罗替卡松更能有效改善肺功能,调节免疫平衡及控制哮喘的发作。Objective： To explore the effect of salmeterol fluticasone combined with montelukast on the lung function and immunologic function of children with asthma and the curative effect.Methods： One hundred children with asthma were randomly divided into the first treatment group and the second treatment group.The first treatment group were treated with salmeterol fluticasone twice daily for 3 months.The second treatment group were treated with salmeterol fluticasone twice daily and 5mg montelukast before sleep daily for 3 months.The lung function was evaluated,the level of IL-4,INF-γ,IgE,EOS were determined and the score of Asthma Control Test was measured before and after treatment.Results： The lung function was significantly improved in the two groups before treatment than after treatment and the scores of ACT were higher.The lung function was improved more significantly in the first treatment group than in the second treatment group and the scores of ACT were higher after treatment.The level of IL-4,IgE,EOS and IL-4/ IFN-γ were higher in the two groups before treatment than after treatment and the level of IFN-γ was lower.The levels of IL-4,IgE,EOS and IL-4/ IFN-γ were higher in the first treatment group than in the second treatment group and the level of IFN-γ was lower after treatment.Conclusion： Salmeterol fluticasone combined with montelukast can improve lung function,regulate immunity and control asthma more effectively than single salmeterol fluticasone.
