摘 要:目的应用血流向量图(vector flow mapping,VFM)技术中的涡流显像模式分析正常成人左室收缩早期心腔内涡流强度连续变化的特点和规律。方法在VFM模式下采集55例健康志愿者标准心尖左室长轴动态彩色多普勒图像,应用2.0版具有涡流强度计算功能的DSA-RS1工作站分析左室腔收缩早期涡流强度的连续变化规律。结果等容收缩期初左室心腔内呈现延续自舒张末期的整体涡流;等容收缩期中仍为整体涡流,涡流形态、位置基本维持不变;至主动脉瓣开放瞬间整体涡流消失,表现为二尖瓣瓣下局部小涡流。涡流强度在左室收缩早期呈现先减小后增大的变化趋势(P<0.01)。结论左室收缩早期心腔内持续出现涡流且规律变化,变化机制尚不明确,其可能与室壁运动及心电传导的时空关联性密切相关。Objective The purpose of this study was to explore the characteristics and regularity of the dynamic changes in vortex intensity in left ventricular early systole phase in normal adults using vector flow mapping (VFM). Methods Fifty normal adults were examined using dynamic color Doppler flow imaging of echocardiography with the mode of VFM. All of images were taken in standard apical left ventricular long-axis view. The changes of vortex in- tensity in left ventrieular early systole phase were analyzed using DSA-RSI workstation (2.0version) which is capable of calculating vortex intensity. Results At the beginning of isovolumic contraction phase, in left ventricle, there was an overall vortex which was the continuation of the overall vortex in end diastole phase and still continued in isovolumic contraction phase with no significant change in shape and position, and then disappeared at the very instant opening of aortic valve, followed by local small vortex below mitral valve. In left ventricular early systole phase, the vortex inten- sity demonstrated dynamic changes characterized by decreasing at first and increasing afterwards (P〈0.01). Conclu- sions We conclude from this study that, in left ventricular early systole phase, there is a continual presence of vortex in left ventricle, with regular changes in intensity and the characteristic of decreasing at first and increasing after- wards; and the underlying mechanism is still unclear by now. Probably, it is related to the space-time correlation be- tween left ventricular wall movement and cardiac electrical conduction.
