摘 要:目的评估实时组织弹性成像对慢性病毒性肝炎肝纤维化分期的诊断价值。方法回顾性分析61例慢性病毒性肝炎患者和20例健康者的弹性参数,计算弹性指数,评估不同程度的肝纤维化分期与弹性指数之间的关系。结果弹性指数与肝纤维化分期具有显著正相关,相关系数r=0.875,(P<0.001)。除S2与S3期的弹性指数之间无显著性差异外,其余各组之间均有显著性差异(P<0.05);S≥S2、S≥S3、S=S4对应的ROC曲线面积分别为0.911、0.904、0.896。结论搭载了组织弥散定量分析功能的实时组织弹性成像对慢性病毒性肝炎肝纤维化分期具有好的诊断价值。Objective This study aimed to evaluate diagnostic value of real-time tissue elastography for differenti- ate different liver fibrosis stages in chronic viral hepatitis. Methods Real-time tissue elastography was performed in 61 patients with liver fibrosis in chronic viral hepatitis and 20 healthy volunteers. 11 parameters were analyzed and elasticity index was calculated. Finally, the correlation between the elasticity index and the liver fibrosis stage was ana- lyzed by corresponding statistical methods. Results The Spearman's correlation coefficient between the elasticity index and the histological fibrosis stage was 0. 875, which is highly significant (P〈0. 001). All comparison of between ex- perimental groups had significant differences except S2&S3 . The outcome indicated that differences between control groups and different fibrosis stages can be distinguished by elasticity indexes. The areas under ROC curve for the diag- nosis of fibrosis S≥S2,S≥S3 and S=S4 were 0. 911, 0. 904 and 0. 896, respectively. Conclusions Real-time tissue elastography with new quantitative technique is a valuable method for the assessment of liver fibrosis in chronic viral hepatitis.
