36例小儿先天性胆总管囊肿的手术治疗

摘　　要：目的:探讨胆总管囊肿切除+肝总管空肠Roux-Y吻合术治疗小儿先天性胆总管囊肿的疗效。方法:回顾性分析1993年1月至2011年3月湖南省湘潭市中心医院收治的36例先天性胆总管囊肿患儿的临床资料,患者均采用胆总管囊肿切除+肝总管空肠Roux-Y吻合术,观察患者病理检查结果、手术时间、术中出血量、手术并发症等指标。结果:36例患者手术时间140～230 min。手术出血量10～50 mL。均痊愈出院,平均住院时间20 d。随访6个月至14年,全组患者未再次手术,B超检查结果未见胆总管狭窄扩张及结石,X线钡餐检查未见钡剂返流到胆道。结论:胆总管囊肿切除+肝总管空肠Roux-Y吻合术是治疗先天性胆总管囊肿的理想方法。Objective： To explore the therapeutic effect of choledochocystectomy plus Roux-Y eholangiojejunostomy in the treatment of children with congenital choledoehal cyst （ CCC ）. Methods： From January 1993 to March 2011,36 child patients with CCC were treated with choledochocystectomy plus Roux-Y cholangiojejunostomy in our ward. The pathological examination results, operating time, intraoperative blood loss and operative complications were observed for all patients. Results ：The operating time for the 36 p/ttients ranged from 140 to 230 minutes and the blood loss ranged from 10 to 50 mL. All cases were cured with an average time of hospitalization of 20 days. Follow-ups, ranging from 6 months to 14 years, revealed no secondary operation. The B-ultrasonic examination showed no bile duct dilatation or stone in the liver. Gastrointestinal barium meal examination showed no reflux in the biliary tract. Conclusion： Combination of choledochocystectomy plus Roux-Y cholangiojejunostomy is an ideal treatment for CCC.
