PVP和PKP治疗骨质疏松性椎体压缩性骨折临床疗效比较

摘　　要：目的对比经皮椎体成形术（PVP）和经皮椎体后凸成形术（PKP）治疗骨质疏松性胸腰椎压缩性骨折的临床疗效，对经皮椎体成形术和经皮椎体后凸成形术的治疗效果进行分析。方法2006年12月-2010年10月共124例在潍坊医学院附属医院脊柱外科行椎体成形术的骨质疏松性胸腰椎体压缩性骨折的病人。随机抽出10例（12个椎体）经皮椎体成形术治疗的病例，为A组；随机抽取10例（14个椎体）经皮椎体后凸成形术治疗的病例，为B组。20例患者共26个椎体，男8例，女12例，年龄50-82岁，平均年龄65岁。随访3个月-2年，平均7．5个月。在疼痛缓解情况、病椎高度恢复率、后凸畸形恢复和并发症方面对两组进行疗效比较。结果两组在疼痛缓解、椎体前缘高度恢复和后凸畸形恢复方面，差异无统计学意义（P〉0．05）。而在椎体后缘高度恢复和并发症方面，差异有统计学意义（P〈0．05）。结论PVP和PKP都能减轻患者疼痛，恢复病椎前缘高度和纠正后凸畸形，但在椎体后缘高度恢复和并发症方面,PKP较PVP有优势。Objective By comparing the PVP group with PKP group of the therapy of osteoporotic thoraeolumbar vertebral. To analysethe treatment effect between PVP and PKP. Methods One hundred and twenty-fuur eases were diagnosed with nsteoporotie thoraco- lumbar vertebral compression fractures from December 2006 to October 2010 in our hospital. Twenty cases, randomly selected from the 124 ca- ses -65 years on average,a total of 26 vertebrae-were divided into two groups. Ten patients （12 vertebrae） in group A accepted the treatment nf PVP for osteoporotic vertebral c.umpression fractures,the other 10 patients （ 14 vertebrae） in group B accepted the treatment of PKP. By comparing the condition of pain relieve, height restoratiun of unhealthy vertebral, restoration of kyphosis Cohb angle and bone cement leakage- related complications in tow groups,to analyse the efficacy between them. Results There was no significant difterence in respect of pain re- lieve, anterior height restoration of unhealthy vertebral and restoration of kyphosis Cobb angle in two groups（ P 〉 0.05 ）. But in respect of the posterior height restoration of unhealthy vertebral and bone cement leakage-related complications, there were significant differences （ P 〈 0. 05）. Conclusion PVP and PKP surgery can both relieve the pain,restore compressed anterior vertebral height and make a significant im- provement in patients with kyphosis, but the PKP group is superior to the PVP group in respecl of posterior height recovery of compressed ver- tebra,reduction of leakage of bone eement and leakage-related complications.
