蝶形减压治疗退行性腰椎管狭窄的疗效分析

摘　　要：目的探讨蝶形减术治疗老年退行性腰椎管狭窄症的疗效。方法自2004年1月～2008年1月共收治51例退行性腰椎管狭窄症患者,26例采用蝶形减压术治疗,25例采用全椎板切除减压治疗;其中合并腰椎不稳10例患者联合应用内固定植骨术;采用日本骨科学会(JOA)15分法对术前和末次随访时的神经功能与自觉症状进行评估,计算改善率。结果随访15个月～5年,平均3.7年,末次随访时JOA评分改善率两组差别无统计学意义,但在手术时间/出血量及平均住院天数上两者差别有统计学意义。结论蝶形减压术是治疗老年退行性腰椎管狭窄症的一种可靠术式,只要把握好手术适应症与减压范围,可获得良好的疗效。Objective To investigate the the butterfly less treatment of degenerative lumbar spinal stenosis efficacy. Methods From January,2004 to January,2008,a total of 51 cases of degenerative lumbar spinal stenosis patients treated,26 patients with butterfly decompression therapy,25 patients with total laminectomy decompression treatment;including Lumbar instability 10 patients combined with internal fixation with bone graft;assessment of the Japanese Orthopaedic Association（JOA） 15 preoperative and last follow-up neurological function with symptoms,to calculate the rate of improvement. Results The follow-up of 15 months to 5 years,an average of 3.7 years,the JOA score improvement rate of the two groups at the last follow-up the difference was not statistically significant,but the difference was statistically significant in operative time / amount of bleeding and the average days of hospitalization. Conclusions The butterfly decompression is a reliable surgical treatment of degenerative lumbar spinal stenosis,as long as a good grasp of the the surgical indications decompression,obtain better efficacy.
