PICC相关性血栓形成后保留导管患者的观察与护理

摘　　要：总结了17例PICC相关性静脉血栓形成后保留导管患者的观察与护理。护理重点包括:严格按医嘱溶栓,严密观察局部症状与体征、继发性出血和肺栓塞等并发症,定期监测出凝血功能和血管超声影像,同时做好血栓肢体的护理,并主动与患者及家属做好沟通,减轻患者心理负担。本组患者均成功保留导管至治疗结束拔管,留管时间为67～189d,无一例血栓再发或加重。This paper summarizes the observation and nursing of 17 patients with peripherally inserted central catheters （P[CC） after the formation of catheter-related venous thrombosis. The key points in nursing were strict administration of throm- bolytic therapy according to doctor＇s orders,intensive observation of local symptoms,signs and complications such as secondary bleeding and pulmonary embolism,regular supervision of coagulation function and vascular ultrasonic imaging,care of aftected extremity,active communication with patients and their families to release their psychological stress. All the cases maintained the PICCs successfully without recurrence or deterioration of thrombosis. The catheter indwelling time ranged from 67 to 189 days.
