益气活血利水法治疗慢性心力衰竭随机对照试验系统评价

摘　　要：目的：评价益气活血利水法治疗慢性心力衰竭的疗效和安全性。方法：采用Coehrane协作网系统评价方法,制订文献纳入标准和排除标准,检索清华期刊全文数据库、维普期刊全文数据库、中国生物医学文献光盘数据库及中国国家知识基础设施等电子数据库;评价纳入文献质量,对同质研究进行Meta分析。结果 ：15项研究被纳入Meta分析,慢性心力衰竭患者共1493例,其中益气活血利水法加常规药物组（治疗组）768例,常规药物组（对照组）725例。Meta分析显示,与对照组相比,治疗组疗效总有效率提高,差异具有统计学意义（OR合并=3.39,其95%CI=2.54~4.53）;治疗组左室射血分数明显高于对照组,差异具有统计学意义（SMD为1.57,其95%CI为0.90~2.24）。结论：现有临床证据表明,在常规药物基础上加用益气活血利水法治疗慢性心力衰竭可能有效,且无严重的不良反应。Objective：To evaluate the efficacy and safety of therapy of replenishing qi,activating blood and promoting urination on treating chronic heart failure by using meta-analysis.Methods：Cochrane systematic review method was used.The inclusion and the exclusion criteria were established based on searching literatures in Tsinghua,VIP,CBMdisc,CNKI and other electronic databases.Then the quality of the included studies was evaluated and meta-analysis was performed on homogeneous researches.Results：Fifteen studies were included in meta-analysis.A total of 1493 cases of patients with chronic heart failure were included.They were divided into herba1（therapy of replenishing qi,activating blood and promoting urination）plus conventional therapy group（768 cases） and conventional therapy group（725 cases）.Meta analysis showed that additional therapy of replenishing qi,activating blood and promoting urination can significantly improve overall efficiency compared with conventional therapy（OR=3.39,95%CI=2.54~4.53）.Left ventricular ejection fraction was obviously elevated in therapy of replenishing qi,activating blood and promoting urination plus conventional therapy group（SMD：1.57,95%CI：0.90~2.24）.Conclusion：Clinical evidence suggests therapy of replenishing qi,activating blood and promoting urination are effective on treating chronic heart failure without serious adverse effects.
