某“三甲”医院2011年手术科室抗菌药物应用情况分析

摘　　要：目的:评价某"三甲"医院抗菌药物的应用特点和趋势。方法:收集该院2011年抗菌药物的应用资料,对其销售金额、用药频度(DDDs)、使用强度(AUD)等进行分析。结果:该院住院患者抗菌药物应用构成比排序列前3位的分别是头孢菌素类(54.18%)、青霉素类(13.62%)、硝基咪唑类(8.29%)。抗菌药物应用率为65.56%,AUD达60.72DDDs/100人/天,高于卫生部规定的60%和40DDDs/100人/天。抗菌药物应用率排序列前10位的科室中除呼吸科外,其余9个均为手术科室;手术科室抗菌药物应用率达78.96%,高于非手术科室的47.89%。结论:该院某些科室存在滥用抗菌药物的现象。为进一步规范抗菌药物的应用,下一步整治重点可以放在手术科室,继续加强对预防性抗菌药物的应用监管,以达到安全、有效、经济用药的目的。OBJECTIVE ： To evaluate the feature and tendency of the utilization of antibacterials in a grade three class A hospi- tal. METHODS： The utilization of antibacterials in the hospital in 2011 were analyzed in respect of consumption sum, DDDs and AUD, etc. RESULTS： Top 3 antibiotics in the list of constituent ratio were cephalosporins （54.18 % ）, penicillins （13.62 % ） and ni- troimidazole （8.29%）. The utilization rate of antibacterials was 65.56% and the AUD of antibacterials in the hospital reached 60.72 DDD/100 persons/day, which were higher than requirement of Ministry of Health （60% and 40 DDD/100 persons/day）. Top 10 de- partments in the list of utilization ratio of antibacterials included department of respiration and other 9 operation departments. The utilization ratio of antibacterials in operation departments reached 78.96%, which was higher than 47.89% of non-surgical depart- ment. CONCLUSION： There are still some departments which have the abuse of antibacterials in the hospital. To further standard- ize the application of antibacterials, the next focus of regulation can be put in surgical department. The prophylactic application of antibacterials should be standardized for a safe, effective and economical goal in our work.
