超声引导胸膜腔注入尿激酶治疗结核性多房性胸腔积液的价值

摘　　要：目的观察超声引导下胸膜腔注射尿激酶治疗结核性多房性胸腔积液的临床疗效。方法对280例多房分隔、胸膜肥厚、包裹性胸膜炎患者经超声引导定位注入10万单位尿激酶,保留24h后再次超声引导下抽取积液。一次效果不理想者重复上述操作,记录每次抽液量及超声表现。结果超声实时监测引导下注入10万单位尿激酶后,182例多房性积液的分隔就全部溶解,抽出液体量明显增多,经重复2-4次注入尿激酶后,65例大部分分隔溶解,有效率为88.2%(247/280)。结论超声在多房性、包裹性结核性胸膜炎的诊断价值大,超声引导定位准确性高,超声引导下胸膜腔注入尿激酶治疗结核性胸膜炎的多房分隔是操作方便、安全有效的辅助诊断治疗方法。Objective Observation ultrasound-guided pleural cavity injection of urokinase treatment of tuberculous multilocular pleural effusion clinical curative effect.Method Of 280 patients with much room space,pleura incrassation,parcels pleurisy patients by ultrasound guide positioning injection 100000 units of urokinase,24 hours later again under the guidance of ultrasound extraction effusion.An effect is not ideal repeats the above operation,record every infusion quantity and sonographic findings.Result Ultrasound real-time monitoring under the guidance of injection 100000 units of urokinase,182 cases multilocular effusion separation were dissolved,extraction liquid volume increased obviously,the repeat 2-4 times injection urokinase,65cases most separation dissolved,effective rate was 88.2%（247/280）.Conclusion Ultrasound in multilocular,parcels sexual tuberculosis pleurisy diagnosis value is big,ultrasound guide positioning accuracy is high,ultrasound-guided pleural cavity injection urokinase treatment of tuberculous pleurisy more room space is easy to operate,safe and effective auxiliary diagnosis and treatment methods.
