慢性阻塞性肺疾病急性加重期患者的生存质量、客观指标及其关系的研究

摘　　要：目的:研究慢性阻塞性肺疾病急性加重期(AECOPD)患者的生存质量、客观指标及其相互间的关系。方法:124例Ⅱ～Ⅲ级AECOPD患者,随机分为实验组(n=62)和对照组(n=62)对照组仅给予常规治疗,实验组在常规治疗的基础上,吸入18μg噻托溴铵,1次/d。结果:两组患者治疗前的生存质量得分及第一秒用力呼气容积占预计值百分比(FEV1%pre),动脉血氧分压(PaO2),C-反应蛋白水平(CRP)比较均差异无显著性(P>0.05);治疗后,与对照组相比,实验组患者的生存质量得分及FEV1%pre,PaO2,CRP均显著性改善(P<0.05);且实验组生存质量评分与FEV1%pre,PaO2有显著相关性。结论:AECOPD患者吸入噻托溴铵后改善了生存质量、FEV1%pre、PaO2、CRP(P<0.05);患者生存质量得分与客观指标FEV1%pre和PaO2成正相关。Objective To Study on the quality of life and objective indicators and relation of them in the patients with acute exacerbation of chronic obstructive pulmonary disease（AECOPD）.Methods 124 COPD cases with a acute exacerbation phase of Ⅱ to Ⅲ COPD,were randomly divided into experiment group and control group.The experiment group had 62 cases with inhaled tiotropium bromide（once-daily 18 ug） based on conventional treatment,and the control group had 62 cases with only conventional treatment.Results There was no significant difference（P 〉 0.05） on the score of quality of life,the first second forced expiratory volume accounted for the expected value ratio（FEV1%pre）,arterial partial pressure of oxygen（PaO2）,and C-reactive protein levels（CRP） in the two groups before treatment.Compared with the control group,the experiment group had significant improvement（P 〈 0.05） with the score of quality of life、 FEV1%pre、 PaO2、 CRP after treatment;and there is a significant correlation between the score of quality of life and FEV1%pre、 PaO2.Conclusion The patients of AECOPD in inhaled tiotropium bromide group had significant improvement（P 〈 0.05） with the score of quality of life、 FEV1%pre、 PaO2、 CRP after treatment ;and the score of quality of life are positive correlation with the FEV1%pre、 PaO2.
