硬膜外阻滞复合全麻在老年患者腹腔镜胆囊手术中的应用

摘　　要：目的：探讨硬膜外阻滞复合全麻在老年患者腹腔镜胆囊手术中的应用。方法：ASAⅠ-Ⅱ级60～85岁胆囊手术患者60例，分成两组：单纯全麻组（Q组，n=30），和硬膜外阻滞复合全麻组（Y组，n=30）。监测SBP、DBP、HR、PETC02、sp02、气道压，记录血流动力学参数，术中所用血管活性药病例数及麻醉药剂量。结果：Y组麻醉诱导、气管插管、切皮时、拔气管导管时SBP、DBP、HR低于Q组，血流动力学稳定，所用麻醉药少于Q组。结论：硬膜外阻滞能不同程度抑制麻醉手术所致的应激反应，硬膜外阻滞复合全麻在老年患者腹腔镜胆囊手术中的应用是安全的、可行的，值得临床推广。Objective： To investigate the epidural block combined with general anesthesia in elderly patients undergo- ing laparoscopic operation. Methods： ASA Ⅰ-Ⅱ 60- 8S years old patients with gallbladder operation in 60 cases, di- vided into two groups： simple general anesthesia group （ group Q, n=30 ）, and epidural block combined with general an- esthesia group （ group Y, n=30 ）. Monitoring of SBP, DBP, HR, PETCO2, SpO2, airway pressure, recording hemo- dynamic parameters, intraoperative use of vasoactive drugs case number and anesthetic dose. Results： in Y group, anes- thesia induction, endotracheal intubation, tracheal tube incision during drawing, SBP, DBP, HR than in the Q group, be modynamic stability, the drug is less than that of Q group. Conclusions： epidural block can be different levels of inhibition of anesthesia operation induced stress response, epidural block combined with general anesthesia in elderly patients under going laparoscopic operation application is safe and feasible, worthy of clinical application.
