摘　　要：目的 :探讨瞳孔改变对急性重型颅脑损伤的临床指导及预后分析 ;方法 :对1 72例急性重型颅脑损伤患者的瞳孔改变进行观察分析 ;结果 :颅内血肿的瞳孔改变占86 2 % ,脑挫裂伤占 42 5 % ,原发性脑干伤及弥漫性轴索损伤占 87 5 % ;而且瞳孔的单双眼改变和对光反射提示不同的临床意义及预后。结论 :急性重型颅脑损伤近 80 %发生瞳孔改变 ,正确观察和判断这些改变 ,对判断病情 ,指导临床治疗和估计预后有重要的临床意见。Objective:To investigate the Clinical significance and prognosis of pupil changes in acute severe head injury patients;Methods:The changes of pupil in 172 cases of acute severe head injury patients were analyzed;Results:The changes of pupil in intracranial hematoma patients is 86.2%,in contusion and laceration of brain patients is 42.5%,in primary injury of brain stem and diffuse axonal injury is 87.5%.The changes of pupil with single or double and pupillary light reflex reflects the degree of illness,judges the prognosis.Conclusion:There are about 80% Changes of pupil in acute severe head injury patients.It has an important clinical significance in evaluating patient's condition,monitoring the clinical treatment and judging prognosis.
