腔镜深筋膜下结扎交通支静脉治疗慢性下肢静脉溃疡

摘　　要：目的　探讨腔镜深筋膜下结扎交通支静脉治疗下肢慢性静脉性溃疡的疗效。方法　手术治疗 7例下肢静脉曲张病人 ,该 7例均同时存在浅静脉倒流、交通支静脉功能不全、深静脉功能不全和静脉性溃疡。手术方式为腔镜深筋膜下交通支静脉结扎、大隐静脉高位结扎并抽剥和小腿曲张的浅静脉经皮连续环形缝扎术 ,其中 5例同时行股浅静脉瓣膜外修复成形术。结果　术后肢体症状和浅静脉曲张消失 ,6例静脉性溃疡短期内愈合 ,1例明显缩小。结论　腔镜深筋膜下交通支静脉结扎术安全有效 ,损伤小 ,并发症少 。Objective To evaluate the role of subfascial endoscopic perforator ligation in treatment of patients with venous ulceration of the leg.Methods Seven varicose patients with venous ulcer in the lower extremities,who had been identified to have reflux in superficial and deep vein as well as incompetent perforating vein,underwent venous surgery.Several surgical procedures were performed simultaneously,including subfasical endoscopic perforator ligation,flush saphenofemoral ligation,limited stripping of the long saphenous and percutaneous continuous venule circumsuture.Five of them underwent external suture valve repair.Results The surgery obviously improved venous disability,varicose vein disappeared;ulcers healed shortly in 6 cases and ulcer reduced markedly in another one case.Conclusion Subfascial endoscopic perforator ligation is a safe and effective technique.It plays an important role in the treatment of varicose and venous ulceration in leg.
