良性前列腺增生并膀胱结石89例的微创治疗体会

摘　　要：目的总结同期微创手术治疗良性前列腺增生合并膀胱结石的经验。方法自2005年3月—2010年11月有选择的对89例良性前列腺增生并膀胱结石患者进行了耻骨上经皮膀胱穿刺造瘘、膀胱结石气压弹道碎石取石、经尿道前列腺气化电切术。结果所有患者均一期治愈,术中出血平均110mL,结石平均手术时间为42min,前列腺气化电切平均手术时间为65min,平均切除前列腺重量41.69g。无膀胱穿孔、结石残余、无大出血,术后排尿均通畅。IPSS评分降至(0.5±2.5)分,MFR升至(19.1±2.3)mL/s。Objective To summarize the experience of minimally invasive surgical treatment for benign prostatic hyperplasia with bladder stones.Methods From March 2005 to November 2010,we had choosed 89 cases of benign prostatic hyperplasia with bladder stones.All of them underwent a percutaneous suprapubic bladder puncture stoma,bladder stones stone lithotripsy,transurethral resection of prostate vaporization surgery.Results All cases were successfully operated with endourological treatment simultaneously.The mean blood loss was 110 mL,mean calculi operating time was 42 minutes,mean transurethral vaporization of prostate operating time was 65 minutes and the average weight of prostate resected was 41.69 g.None had complications such as hemorrhea,vesical perforation and other life-threatening complications.All patients were withdrawn Foley catheter and voiding easy and smooth.IPSS,MFR and Residual urine were evidently improved after operation.Conclusion Suprapubic cystostomy URSL combined TURP treatment of senile prostatic hyperplasia with bladder stones is a minimally invasive and effective treatment method for BPH and bladder stones
