摘　　要：目的评价手术治疗胫骨远端Pilon骨折的效果,探讨并发症的防治措施。方法对2003年12月～2011年12月手术治疗的Pilon骨折52例进行随访分析。根据Ruedi和Allogwer分类方法,Ⅰ型26例,Ⅱ型14例,Ⅲ型12例。结果 52例术后均获随访,按Mazur的标准进行疗效评定,优良率76.9%。结论对于Pilon骨折的治疗除了关注骨折本身的复位,更应重视创伤周围软组织损伤程度及术中对软组织的保护,早期有限内固定或外固定及延迟切开复位内固定应作为治疗高能量胫骨Pilon骨折的首选。良好的对位对线复位及坚强的内固定,可预防关节僵硬和创伤性关节炎的发生。Objective To observe the methods of Pilon fracture and the effect,as well as the prediction and treatment to the complications.Methods In 2003 December 2011 December operation treatment in 52 cases of Pilon fracture follow-up analysis.According to Ruedi and Allogwer classification methods,Ⅰ 26 cases,type Ⅱ 14 cases,type III in 12 cases.Results In all followed-up 52 patients,76.9% of them were satisfactory.Conclusion The extend of the injury of the soft tissue should be put the same emphasis on,as well as the reduction and fixation to the fracture when treating the Pilon fracture.Early limited internal fixation or external fixation,then delayed ORIF should be the first choice to Pilon fracture.Anatomical reduction,rigid internal fixation and early full range movement of ankle joint can prevent the ankylosis and traumatic arthritis.
