C反应蛋白、白细胞计数、中性粒细胞百分比与阑尾炎病理类型关系

摘　　要：目的探讨急性阑尾炎患者血清C反应蛋白、白细胞计数、中性粒细胞百分比与病理类型关系及C反应蛋白、白细胞计数、中性粒细胞百分比对急性阑尾炎的诊断价值。方法入院时测定所有行手术治疗的急性阑尾炎患者血清C反应蛋白、白细胞计数、中性粒细胞百分比,手术后将患者按手术病理类型分组,观察各组患者上述指标,比较各组数据有无统计学差异,并根据患者病理类型分别与C反应蛋白、白细胞计数、中性粒细胞百分比进行相关性分析。同时比较不同病理类型患者C反应蛋白、白细胞计数、中性粒细胞百分比阳性率。结果随着急性阑尾炎患者病理程度的加重,血清C反应蛋白、白细胞计数、中性粒细胞百分比均上升,差异具有统计学意义。患者血清中性粒细胞百分比与病理程度显著相关,白细胞计数与病理程度低度相关,而C反应蛋白与病理程度弱相关。各组患者血清白细胞计数、中性粒细胞百分比、C反应蛋白阳性率均无统计学差异。结论患者血清白细胞计数、中性粒细胞百分比较C反应蛋白在反应患者阑尾炎症程度上具有优势,C反应蛋白、白细胞计数、中性粒细胞百分比对急性阑尾炎患者诊断价值无明显差异。Objective： To analyze the correlation between C-reactive protein, leukocyte count, percentage of neutrophils and pathologic type in acute appendicitis patients, and the value of C-reactive protein, leukocyte count, percentage of neu- trophils in the diagnosis of acute appendicitis. Methods：All the patients undergone appendectomy in the Department of Gen- eral Surgery, the Affiliated Jiangning Hospital of Nanjing Medical Universit were divided into 3 groups according to the pathologie type of appendix. The levels of C-reactive protein,leukocyte count and percentage of neutrophils in plasma be- tween the 3 groups were compared by the analysis of Variance, and the correlation between C-reactive protein, leukocyte count, percentage of neutrophils and pathologic type was analyzed. We also analyzed the value of C-reactive protein, leuko- cyte count, percentage of neutrophils in the diagnosis of acute appendicitis. Results： The levels of C-reactive protein,leu- kocyte count and percentage of nentrophils in plasma were significantly different between the 3 groups, and percentage of neutrophils was obviously correlated with the pathologic type of appendix. There was no statistical difference in C-reactive protein, leukocyte count, percentage of neutrophils in the diagnosis of acute appendicitis. Conclusion： Leukocyte count and percentage of neutrophils can evaluate the severity of acute appendicitis better than C-reactive protein. There is no differ- ence in C-reactive protein, leukocyte count, percentage of neutrophils in the diagnosis of acute appendicitis.
