河南省手足口病重复感染及影响因素分析

摘　　要：目的了解河南省手足口病的重复感染率、流行特征和影响因素。方法筛选《中国疾病监测信息报告管理系统》中河南省2009年首次感染手足口病并在2010年底之前发生重复感染的病例资料,应用SPSS13.0,采用描述性和分析性统计学方法进行分析。结果河南省手足口病的重复感染率为1.14%。首次感染和第二次感染的时间曲线基本一致,高峰均在4月份;重复感染率的地区分布差异有统计学意义(χ2=332.9355,P<0.0001);男性重复感染率(1.2%)高于女性(1.03%)(χ2=6.5237,P<0.0001);随着年龄的增长,重复感染率呈下降趋势(Z=-23.8574,P<0.0001);城市儿童重复感染率高于农村(χ22=7.57,P=0.0059)。年龄是手足口病重复感染的保护因素(OR=0.735,95%CI:0.591-0.913),农村亦是其保护因素(OR=0.463,95%CI:0.287-0.748)。结论应关注手足口病重复感染状况,尤其是对高危地区和高危人群,应采取针对性的防控措施。Objective To analyze the incidence of repeated infection,epidemiological characteristics and influencing factors of hand-foot-mouth disease（HFMD）in Henan.Methods The data of HFMD cases infected in 2009 and reinfected before 2010 in Henan were collected from national diseases reporting information system and analyzed by usisng descriptive and statistical methods.Results The repeated infection rate of HFMD in Henan province was 1.14%.The time curves of primary infection and repeated infection were generally consistent and the peak times with peak in April.The repeated infection rates of HFMD among different areas were statistically significant difference（χ2=332.9355,P0.0001）.The repeated infection rate of HFMD in males（1.2%） was higher than that of females（1.03%）（χ2=6.5237,P0.0001）;The repeated infection rate decreased with age（Z=-23.8574,P0.0001）;The repeated infection rate in urban area was higher than that in rural area（χ2= 7.57,P=0.0059）.Age was a protective factor for repeated infection of HFMD（OR=0.735,95%CI：0.591-0.913） and rural area was also a protective factor（OR=0.463,95%CI：0.287-0.748）.Conclusion Effective measured be adopted for prevention and control of HFMD in high risk areas and populations.
