糖化血清蛋白与胎儿发育及代谢轴关系的研究

摘  要:目的:通过比较小于胎龄儿(SGA)与适于胎龄儿(AGA)的脐血糖化血清蛋白(GSP)水平,探讨GSP与胎儿发育及代谢轴之间的关系及检测新生儿GSP的意义。方法:以25例SGA为病例组,35例AGA为对照组,测量两组新生儿体格发育指标,检测脐血GSP水平及胰岛素、胰岛素样生长因子-1(IGF-1)等代谢轴相关因素并进行比较。结果:SGA组GSP、胰岛素、IGF-1水平低于对照组,组间差异均具统计学意义(P<0.05);GSP水平与出生体质量、身高、头围、BMI等体格发育指标呈正相关(r=0.632、0.676、0.628、0.432,P均<0.05)。结论:SGA宫内血糖平均水平低于AGA,并同时存在低胰岛素及低IGF-I状态;GSP可以作为反映胎儿宫内发育情况及营养状态的指标。Objective: To study the Correlation between GSP and intrauterine fetal growth, metabolism axis by comparing the GSP concentration of small for gestational age (SGA) and appropriate for gestational age (AGA). Methods: Twenty-five cases of SGA and 35 cases of AGA were enrolled, and the latter as normal control group. The cord blood level of GSP, insulin and IGF-1 as well as the birth weight and length, head circumference and BMI were measued. Results: GSP, insulin and IGF-lconcentration of SGA was lower than those of AGA (P 〈 0.05 ). GSP concentration was positively related to birth weight, birth length, head circumference, body mass index (BMI), all P 〈0. 05. Conclusions: SGA has lower level of GSP, insulin, IGF-1 than AGA, indicating that SGA should have lower intrauterine blood glucose. GSP could reflect the intrauterine fetal development and nutritional status.
