化瘀涤痰方治疗慢性硬膜下血肿临床研究

摘　　要：目的:观察化瘀涤痰方对慢性硬膜下血肿(CSDH)的治疗作用。方法:将58例CSDH患者随机分为治疗组31例和对照组27例。采用多田氏法计算2组初诊和每疗程的血肿量变化,同时观察2组初诊和每疗程眩晕及肢体麻木症状变化的情况。结果:总有效率治疗组为87.10%,对照组为51.85%,2组比较,差异有非常显著性意义(P<0.01)。对照组治疗前后血肿量变化比较,差异有非常显著性意义(P<0.01);治疗组治疗前后血肿量变化比较,差异有非常显著性意义(P<0.01)。2组间治疗后血肿量比较,差异有非常显著性意义(P<0.01)。治疗组眩晕症状改善率为83.33%,对照组为57.89%,2组比较,差异有非常显著性意义(P<0.01)。治疗组肢体麻木症状改善率为88.89%,对照组为50.00%,2组比较,差异有非常显著性意义(P<0.01)。结论:化瘀涤痰方治疗CSDH,可在一定程度上加快血肿吸收,改善患者眩晕及肢体麻木症状。Objective： To observe the efficacy of Huayu Ditan Recipe（HDR） a Chinese herbal prescrip- tion with the action of eliminating phlegm and blood-stasis, for the treatment of chronic subdural hematoma（CSDH）. Methods： A total number of 58 patients in 3 hospitals were randomly assigned into two groups, 31 in the treatment group, and 27 in the blank control group. Duotian＇s method was used to cal- culate the changes of hemotoma volume in the two groups at the first visit and at the end of each course. Meanwhile, the development of vertigo and limb numbness were also observed. Results： The total effec- tive rate in the treatment group was 87.10% and was 51.85% in the blank control group, showing signifi- cant difference between the two groups（P〈 0.01）. After treatment, the hemotoma volume was decreased both in the blank control group（P〈 0.01） and the treatment group（P〈 0.01）. The decrease of hemotoma vol- ume in the treatment group differed from that in the control group（P〈0.01） after the treatment. The im- provement rate of vertigo was 83.33% and 57.89%, and the total improvement rate of limb numbness was 88.89% and 50.00% in the treatment group and the blank control group, respectively, the differ- ences being significant between the two groups（P〈0.01）. Conclusion： Huayu Ditan Recipe is effective in the treatment of CSDH by promoting the absorption of hematoma and by relieving vertigo and limb numb- ness to certain extent.
