失效模式和效应分析在截瘫患者压疮预防流程的应用研究

摘　　要：目的探讨失效模式和效应分析(FMEA)在截瘫患者压疮预防流程的应用效果。方法应用FMEA对截瘫患者引起压疮的失效模式进行分析、评估,找出截瘫患者引起压疮的原因,通过计算危机值(RPN)提出问题,并制定相应的整改措施。结果实施整改后截瘫患者压疮预防流程的风险RPN下降了65.3%,压疮发生率为零(P<0.05)。结论 FMEA作为一种风险管理工具,应用于压疮预防流程,能在压疮发生前进行有效预测、评估,采取有效的措施,降低压疮发生率。Objective Purpose： Investigate the application effects of failure mode and effects analysis （FMEA） on ulcer pressure prevention process of paraplegic patients. Methods Analyzed and evaluated failure model of pressure uieer in paraplegic patients with FMEA, discovered the reasons of pressure ulcer, identified the problem and corresponding improvement measures by calculating and analyzing RPN. Results The risk RPN value of pressure ulcer prevention decreased by 65.3%, and the pressure ulcer incidence was 0 in paraplegic patients after the improvement measures were carried out. Conclusion As arisk control tool in pressure ulcer prevention process, FMEA can be used to forecast and evaluate before the occurrence of pressure ulcer, take effective measures to reduce the incidence o{ pressure ulcers.
