单孔腹腔镜与常规腹腔镜胆囊切除术后切口的比较研究

摘　　要：目的对比单孔腹腔镜胆囊切除术（SILC）与常规腹腔镜胆囊切除术（CLC）术后切口的情况。方法 2011年1~6月,对40例施行SILC（SILC组）和同期配对施行CLC（CLC组）的患者,分别于术后第2天和术后1个月门诊随访时检查切口情况,并填写问卷。结果术后第2天和术后1个月随访时,两组患者切口疼痛程度相近,差异无统计学意义（P〉0.05）。术后第2天切口明显程度、满意程度SILC组优于CLC组（P〈0.05）。术后1个月随访时,SILC组仍高于CLC组,但差异无统计学意义（P〉0.05）。术后1个月随访时,两组患者切口满意程度均较术后第2天升高,但仅CLC组升高有统计学意义（P〈0.05）。结论单孔腹腔镜手术较常规腹腔镜手术具有瘢痕隐蔽及由此引起的切口满意度高的优势,但这些优势随时间推移而消失。就术后切口疼痛程度而言,SILC并无优势。Objective To compare the patient＇s perception of cosmetic outcome after single-incision laparoscopic cholecystectomy （ SILC ） with conventional laparoscopic cholecystectomy （CLC）. Methods The SILC and CLC patients, who were operated in the last years, were recruited for personal interview and clinical assessment at second day and one month after operation by using questionnaire. The satisfaction with the cosmetic appearance of scars and the scar-related symptoms was assessed in all the patients. Results Eighty patients were included in the study （ 40 SILC v.s. 40 CLC ）. The scar-related symptoms were similar in both groups and at both time points （ two days later and one month later ）. The scores of visible scars and satisfaction with the appearance were statistically different in both groups （ 3.30 ± 0.61 in SILC v.s. 2.83 ± 0.64 in CLC , P = 0.001 . 4.03 ± 0.48 in SILC v.s. 3.75 ± 0.54 in CLC , P = 0.019 ） at second day, but there was no difference one month later. The scores of satisfaction were higher one month later compare to the second day, only difference of CLC group was significant（ P 0.05 ）. Conclusions The patient perception regarding scar-related symptoms after SILC and CLC was similar in both groups. The main and short term benefit to the patient underwent the SILC operation may be scar hidden only, and the higher satisfaction maybe associated with it.
