摘 要:目的探讨慢性阻塞性肺疾病(cOPD)发展过程中肺气肿的变化规律并找出肺气肿CT测量量化指标与肺功能分级的关系。方法对比2009—12—2011-06在广州医学院第一附属医院诊治并行胸部CT的COPD病例90例(肺功能轻度16例,中度36例,重度38例)及对照组45例的CT图像,测量2组病例肺气肿程度的异同,并分析COPD病例中肺功能轻、中、重度三亚组中肺气肿的差别。结果COPD病例的肺容积平均为(4056.12±64.20)cm3,肺气肿容积平均为(1011.80±15.5)cm3,肺气肿/肺容积比值为(21.6±6.0)%;COPD病例的肺容积、肺气肿容积和肺气肿/肺容积比值均较对照组明显增大,2组间的差异有统计学差异(P〈0.05)。COPD病例随着肺功能下降程度的加重,肺容积、肺气肿容积和肺气肿/肺容积比值均有递增趋势,经方差分析,三者在肺功能轻度、中度、重度三亚组间两两对比的差异均有统计学差异(P〈0.05)。结论CT容积测量技术能有效监测COPD肺气肿变化情况,随着COPD的进展,肺气肿程度不断加重,肺气肿变化与肺功能分级具有一定相关性。Objective To investigate the change of pulmonary emphysema in the development of chronic obstructive pulmonary disease (COPD) and evaluate the relationship between pulmonary function and CT volume measurement of pulmonary emphysema. Methods A volume CT scan was performed in 135 participants, including 90 patients with COPD demonstrated clinically and by pulmonary function measurement (COPD group) and 45 patients with normal lung (control group) in our hospital. The COPD group was divided into three subgroups depending on their pulmonary functions , including 16 cases in mild-range , 36 cases in mid range, 38 cases in severe-range. The volume of pulmonary emphysema and the lungs were measured, and the difference of pulmonary emphysema among the groups was analyzed. Results The lung volume was (4 056. 12±64. 20) cm3 and the pulmonary emphysema volume was ( 1 011.80 ± 15.5 ) cma in the COPD group. The ratio of pulmonary emphysema volume to lung volume in the COPD group was ( 21. 6 ± 6.0 ) %. There was statistical significance between the COPD group and the control group in the lung volume, the pulmonary emphysema volume and the ratio of pulmonary emphysema volume to lung volume, with the COPD group having greater values. Lung volume, pulmonary emphysema volume and the ratio of pulmonary emphysema volume to lung volume increased gradually from mild range to mid-range to severe range subgrpups, and there was statistical significance among the three subgroups. Conclusion CT volume measurement can monitor the change of pu]monary emphyscma in COPD. Pulmonary emphysema turns more severe with the development of COPD, and the change of pulmonary emphysema correlates with the grade of pulmonary function.
