摘  要:目的探讨3.0T磁共振扩散张量纤维束成像(diffusiontensortractography,DTT)对正常前列腺纤维束空间结构显示的能力,并评估观察测量正常前列腺的外周带及中央叶各向异性(fractionalanisotropy,FA)值及表观扩散系数(apparentdiffusioncoefficient,ADC)值。方法11例健康志愿者平均年龄为30岁(年龄范围25~44岁),既往无前列腺疾病史。采用3.0TMR扫描仪,6通道腹部相控阵线圈,行轴位T2WI加脂肪抑制扫描及轴位扩散张量成像(diffusiontensorimaging,DTI)扫描,在Neuro3D软件中用“种子法”进行前列腺纤维束追踪,评估其价值,“感兴趣区(regionsofinterest,ROD法”测量外周带及中央区FA值及ADC值,评估其差异性。结果DTT能很好显示正常前列腺纤维束走行、排列特点。在所有志愿者外周带及中央叶之间FA值及ADC值差异均有统计学意义,中央叶ADC平均值(1.032±0.132)×10^-3mm2/s显著低于外周带ADC平均值(1.505±0.187)×10^-3mm2/s(P〈0.01),外周带FA平均值0.1626±0.0227显著低于中央叶FA平均值0.2385±0.0432(P〈0.01)。结论DTT能较好显示正常前列腺纤维束(平滑肌及弹性纤维)的空间结构特点,FA值及ADC值的高低可反映前列腺细微组织结构差异,提示DTT及DTI在前列腺疾病诊断方面将可能提供重要的信息。Objective To assess the ability of 3T MR diffusion tensor tractography (DTT) to show the spatial structure of the prostate fibers, and to determine the normative fractional anisotropy (FA) and apparent diffusion coefficient (ADCI value of healthy prostate. Methods Eleven volunteers (age 25--44 years, mean 30) with no history of prostate disease underwent 3T MRI and dif fusion tensor imaging{DTI) using a 6-channel external phased-array coil. T2 weighted turbo spin-echo (TSE} axial images and DTI with single-shot echo planar imaging of the prostate were obtained. Using Neuro 3D software, the seed point of the interesting re gion was given and the prostate fibers were traced. Regions of interest were defined in the peripheral zone (PZ) and central zone {CZ), and ADC and FA values were determined. Differences between parameters of the central and peripheral zones were assessed. Results DTT clearly showed the spatial structure of the prostate fibers. There was statistically significant difference in ADC and FA values between central and peripheral zones. The mean ADC value of the CZ (1. 032 ×10^-3mm2/s±0. 132 ×10^-3mm2/s) was sig- nificantly lower than that of the PZ {1. 505×10^-3mm2/s ±0. 187×10^-3mm2/s, P〈0.01}. The mean FA value of the PZ {0. 162 6 ±0. 022 7) was significantly lower than that of the CZ (0. 238 5±0. 043 2, P〈0.01). Conclusion DTT can show the spatial struc- tural characteristics of the normal prostate fibers. The values of FA and ADC can reflect the difference of fine structure organization in the prostate. DTT and DTI may provide important information in the diagnosis of prostatic diseases.
