摘 要:目的调查住院冠心病患者的糖代谢状况,为强化糖代谢异常的治疗提供依据。方法选择经冠状动脉造影确诊的住院冠心病患者72例,常规检测空腹血糖(FPG)、糖化血红蛋白(HbAlc)。对FPG未达糖尿病诊断标准的无糖尿病病史者进行口服葡萄糖耐量试验(OGTr)。结果72例患者中糖代谢异常48例(66.7%),糖尿病患者29例(40.3%),包括有确诊糖尿病病史者19例(26.4%),经入院后FPG检查确诊的新发糖尿病患者4例(5.6%),经OG'Iq'检测确诊的糖尿病患者6例(8.3%);空腹血糖受损(IFG)2例(2.8%),糖耐量异常(IGT)17例(23.6%);其余24例(33.3%)糖代谢正常。糖代谢异常的知晓率为31.9%。在有糖尿病病史的19例患者中,HbAlc控制理想者仅3例(15.8%),控制良好者10例(52.6%),6例(31.6%)控制差。糖代谢异常组高脂血症的发生率显著高于糖代谢正常组(66.7%VS.16.7%,P〈0.05)。结论近2/3的住院冠心病患者有糖代谢异常,其知晓率不及1/3,已知糖尿病患者的血糖控制率低,因而对冠心病患者糖代谢异常的诊治有待提高。Objective To investigate the glycometabolism status in patients hospitalized for coronary artery disease (CAD) , and offer evidence for enforcing the treatment of glycometabolism disorders. Methods 72 hospitalized patients, diagnosed as CAD by coronary artery angiography, were consecutively recruited. Serum levels of fasting plasma glucose (FPG), glycosylated hemoglobin (HbAlc) were routinely detected, and oral glucose tolerance test (OGTY) was applied in patients who had no history of diabetes and did not meet the diagnosing criteria of diabetes by FPG. Results Among the 72 patients, 48 (66.7%) were complicated with glycometabolism disorders, in which 19 (26.4%) had known histories of diabetes, 4 ( 5.6% ) were diagnosed as new onset diabetes by FPG ; 6 ( 8.3 % ) as new onset diabetes by OGTY, 2 (2.8%) as impaired fasting glucose ( IFG), 17(23.6% ) as impaired glucose tolerance (IGT) ; and the remaining 24 (33.3%) patients had normal glycometabolism. The HbAlc was ideally controlled in 3 (15.8%), well controlled in 10 (52.6%) , and poorly controlled in 6 (31.6%) of the 19 patients were known as diabetes. The morbid- ity of hyperlipemia in patients with glyeometabolism disorder was significantly higher than those with normal glycome- tabolism (66.7% vs. 16.7% , P 〈 0.05 ). Conclusion Nearly two - thirds of hospitalized CAD patients are complicated with glycometabolism disorders, which is known in less than one - third of the patients. The ratio of ideal glucose control is low in patients with known diabetes. Improving the diagnoses and treatment of glycometabolism disorders for CAD patients is needed.
