实时三维超声心动图评价原发性高血压病患者左心房容积和收缩功能的临床价值

摘　　要：目的探讨实时三维超声心动图(RT-3DE)评价原发性高血压病(EH)患者左心房容积和收缩功能的临床价值。方法将EH患者分为左心房大小正常组(A组)40例、左心房增大组(B组)40例及正常对照组40例,应用RT-3DE测量左心房最大容积(LAVmax)、左心房最小容积(LAVmin)、左心房每博射血量(LASV)、左心房射血分数(LAEF)、左心房被动排空分数及左心房主动排空分数,并对两组结果进行对比分析。结果与正常对照组比较,A组LAEF和左心房主动排空分数增大,被动排空分数减低,差异有统计学意义(P﹤0.05);B组LAVmax、LAVmin、LAEF、LASV及左心房主动排空分数增大,被动排空分数减低,差异有统计学意义(P﹤0.05)。与A组比较,B组LAVmax、LAVmin及LASV增大,差异有统计学意义(P﹤0.05)。结论 RT-3DE评价左心房容积和收缩功能准确可靠,能准确反映EH患者左心室舒张功能受损的程度,可为其左室舒张功能减低提供诊断依据。Objective To explore the clinical value of real-time three-dimentional echocardiography （RT-3DE） in the evaluation of left atrial volume and function in patients with essential hypertension （EH）.Methods One hundred and twenty two subjects were involved in this study, including 40 EH patients with normal left atrial （group A） ,40 EH patients with increscent left atrial （group B） and 40 healthy subjects （control group）.RT-3DE technology was applied to measure the maximum left atrial volume （LAVmax）,the minimum left atrial volume （LAVmin）,the left atrial volume change value,left atrial ejection fraction （LAEF） ,left atrial active ejection fraction （LAAEF） and left atrial passive ejection fraction （LAPEF）.Results The LAAEF in group A was increased but LAPEF was decreased as compared to those in the control group （P 〈 0.05）. The LAVmax,LAVmin, LAEF, LASV and LAAEF in group B were increased but LAPEF was decreased as compared to those in the control group （P 〈 0.05）. The LAVmax, LAVmin and LASV in group A were bigger than those in group B （P 〈 0.05 ）.Conclusion RT-3DE technology can accurately and reliably assess the left atrial volume and function and correctly reflect the left ventrieular diastolic dysfunction in patients with primary hypertension, which providing evidence for the diagnosis of left ventricle diastole function.
