组织多普勒成像技术评价冠心病患者心脏舒张功能的临床意义

摘　　要：[目的]应用组织多普勒成像(DTI)技术评价不同程度冠心病患者的心脏舒张功能,并探讨其临床意义.[方法]对30例健康对照者及40例患有不同程度冠状动脉狭窄的患者分别行常规多普勒及DTI检查,根据冠状动脉狭窄的程度分为异常A组(22例狭窄率超过50%)和异常B组(18例狭窄率不足50%).应用常规多普勒技术主要测量二尖瓣口早期充盈速度E峰(E)和舒张晚期充盈速度A峰(A)及E/A比值;DTI检查主要测量左心室等容舒张时间(IVRT)、二尖瓣环舒张早期运动速度(Ve)、舒张晚期运动速度(Va)和Ve/Va比值,并对所得数据进行对比研究.[结果]异常组Ve,Ve/Va较对照组明显降低(P<0.05),异常组IVRT较对照组明显延长(P<0.05);异常A组IVRT较异常B组明显延长(P<0.05);异常B组Ve,Ve/Va较对照组显著降低(P<0.05);异常B组的IVRT较对照组显著延长(P<0.05).[结论]DTI技术是评价不同程度冠心病患者左心室舒张功能的可靠方法.OBJECTIVE To evaluate the cardiac diastolic function in different degree of patients with coronary heart disease by using tissue Doppler imaging （DTI）, and to study its clinical significance. METHODS 30 cases of healthy controlers and 40 patients with different degree of coronary stenosis were examined by routine Doppler and DTI, respectively, and according to the degree of coronary stenosis, the subjects were divided into abnormal group A （22 cases of stenotic rate were higher than 50%） and B（18 cases of stenotic rate were less than 50% ）. The E, A and E/A ratio were detected by routine Doppler, and the IVRT, Ve, Va and Ve/Va ratio were detected by DTI, and all of data were comparatively analyzed. RESULTS The Ve and Ve/Va ratio were lower in abnormal group than in control group （P d0.05）, the IVRT was significantly prolonged in abnormal group than in control group （P 〈 0.05）, the IVRT was significantly prolonged in abnormal group A than in abnormal group B（P〈0.05）, the Ve and Ve/Va ratio were significantly lower in abnormal group B than in control group （P 〈 0.05）, and the IVRT was significantly prolonged in abnormal group B than in control group （P 〈 0. 05）. CONCLUSION DTI is a reliable method to evaluate cardiac diastolic function in patient with different degree of coronary heart disease.
