凝聚胺法配血不合的原因分析及对策

摘　　要：目的为了解疑难配血类型,分析交叉配血不合的原因,来指导临床安全合理用血。方法采用凝聚胺法进行交叉配血,11年里在河南省胸科医院共发现55例交叉配血不合的现象,现对这些病例进行分析总结,并结合病史进行相关讨论,找出相应的处理方法。结果引起患者交叉配血不合的原因较多,其中不规则抗体引起占29.1%,冷凝集引起占23.6%,血浆蛋白含量过高引起占14.5%,ABO亚型引起占7.3%,自身抗体引起占7.3%,抗体减弱引起占7.3%,药物引起占1.8%。结论交叉配血不合的影响因素有标本、温度、疾病、输血史、妊娠史、大剂量抗菌药物史等,发现交叉配血不合,要分析原因找到处理办法,以保证患者输血安全;另外提倡自身输血和成分输血,尽量减少异体输血,是减少配血不合的最佳方法。Objective To understand the type of difficult match blood not cross matching, analyze the reasons, to guide clinical safety reasonable use. Methods Using brine with blood method and condensing amine method with blood cross matching blood, in 1 l years, 55 patients in our hospital were found not cross matching the phenomenon of blood, these cases were analyzed and summarized in history, and combining the related discussion, find out the corresponding pro- cessing method. Results Cause patients not cross matching blood the reason more, including the same immune antibody 29.1% , cold agglutination caused of 23.6% condensing caused by integrating cause of plasma protein content 14. 5% , high ABO subtypes of 7. 3%, autoantibody induced by 7.3% , antibody weakened caused of 7. 3%, drug - induced caused of 1.8%. Conclusions The influence factors of Cross matching blood not are specimens, temperature, disease, blood transfusion history, pregnancy and history, history of high - dose antibacterial drugs, such as un - islamic, cross matching blood found analyze its causes find, to ensure the treatment method of patients＇ blood transfusion safety; An-other advocate their blood transfusion and component transfusion, minimize blood transfusions, allograft are the best method to reduce blood disagreement.
