慢性浅表性胃炎脾虚证胃黏膜细胞凋亡、环氧合酶-2表达的意义

摘　　要：目的:探讨慢性浅表性胃炎(脾虚证)与细胞凋亡、环氧合酶-2的表达及意义。方法:选择脾虚证、脾胃湿热证的慢性浅表性胃炎患者与正常对照组作为研究对象,各组例数分别为20例、20例、10例,均行电子胃镜下钳取胃黏膜组织,用原位末端标记法检测胃黏膜细胞凋亡指数,采用免疫组织化学法检测胃黏膜组织中COX-2的蛋白表达。结果:脾虚证组、脾胃湿热证组的细胞凋亡指数高于正常对照组,脾虚证组的细胞凋亡指数高于脾胃湿热证组,差异均有统计学意义(P<0.01);COX-2蛋白表达脾虚证组、脾胃湿热证组强于正常对照组;脾胃湿热证组强于脾虚证组,差异均有统计学意义(P<0.01))。结论:慢性浅表性胃炎脾虚证胃黏膜细胞凋亡明显增加,脾胃湿热组胃黏膜组织中COX-2蛋白表达明显增高。慢性浅表性胃炎胃黏膜组织中细胞凋亡、COX-2蛋白表达,是脾虚证和脾胃湿热证候形成的分子生物学特点之一。Objective：To study the relationship between the superficial gastritis（spleen deficiency）,epoxy synthase-2 and the cell apoptosis.Method：Choose the patients with chronic superficial gastritis of spleen deficiency,spleen and stomach of damp and hot card and normal control group as the research objects,the case numbers of each control group are 20,20 and 10.All of the gastric mucosal tissue are collected in situ by the protractors of electronic gastroscope;the stomach mucosa cell apoptosis index is detected by end labeling method,and the COX-2 protein expression of gastric mucosa tissue is detected by immunohistochemical method.Results：The cell apoptosis index of spleen deficiency group,spleen and stomach of damp and hot card group are higher than that of normal control group,and the differences are significant（P0.01）.The cell apoptosis index of spleen deficiency group is higher than that of the spleen and stomach of damp and hot card group（P0.01）.The COX-2 protein expression of spleen deficiency group and spleen and stomach of damp and hot card group is stronger than that of normal control group（P0.01）.The COX-2 protein expression of the spleen and stomach of damp and hot card is stronger of than that of spleen deficiency group（P0.01）.Conclusion：The mucosa cell apoptosis of spleen deficiency stomach is increased obviously,and the COX-2 protein expression of the stomach mucosa tissue of spleen and stomach of damp and hot card is increased obviously.The mucosa cell apoptosis and the COX-2 protein expression of stomach mucosa tissue of superficial gastritis is a molecular biology feature of the formation of spleen deficiency and spleen and stomach of damp and hot card.
