摘  要:目的比较三种髓外内固定方法治疗老年股骨粗隆间骨折的疗效。方法对96例接受髓外内固定方法治疗的老年股骨粗隆间骨折患者的临床资料进行回顾性分析,96例中36例采用动力髋螺钉(DHS组),28例采用对侧股骨远端锁定钢板(DF—LCP组),32例采用股骨近端解剖型锁定钢板(PF—LCP组)。比较三组手术时间、手术出血量、术后卧床时间、骨折愈合时间、髋关节功能评分及并发症情况。结果DHS组的手术时间、手术出血量多于其他两组(P〈0.05),而DF—LCP组与PF—LCP组比较差异无统计学意义。术后卧床时间DHS组相对较短,而三组总体比较差异无统计学意义。髋内翻等并发症的发生率,DHS组、DF—LCP组和PF—LCP比较差异有统计学意义(P〈0.05)。三组间骨折愈合时间、髋关节功能恢复情况比较差异无统计学意义。结论三种内固定方法均是治疗老年股骨粗隆间骨折的有效方法,以股骨近端解剖型锁定钢板最佳,PF—LCP具有手术时间短、出血少、并发症少等优势。Objective To compare the effects of surgical treatment with three kinds of internal fixation methods on senile intertroehanteric fracture. Methods The clinical data of 96 eases of senile in- tertroehanteric fracture admitted from March 2006 to October 2010 were reviewed retrospectively. Proxi- mal femoral fixation with dynamic hip screw( group DHS)in 36 cases, distal femoral locking plate( group DF-LCP) in 28 cases and proximal femoral locking plate( group PF-LCP) in 32 cases. Perioperaive inde- xes of patients were statistically analyzed, including operative time, blood loss, healing time of fracture, the function evaluation and cemplications. Results The reduced operative time andblood loss were higher in DHS group, compared with other two groups, there was no significant difference in healing time of fracture and the function evaluation between the three groups. Group PF-LCP had fewer complications compared with the other groups. Conclusions The three internal pxation methods are all effective methods for se- nile intertrochanteric fracture, and PF-LCP is the best internal fixation method. It has advantages of shor- ter operative time, less blood loss, fewer complications and so on.
