摘  要:目的:本研究拟比较机械通气患者镇静时平均吸气压(Pi)和食道压力(Peso)的相关性与一致性。方法:12例ICU机械通气患者,通过插入食道囊管监测Peso,靶控输注异丙酚进行镇静,测定气道阻断压(P0.1)、吸气时间(Ti),计算平均吸气压(Pi),分析P0.1及Pi与Peso之间的相关性。结果:P0.1与Peso之间以及Pi与Peso之间的相关系数分别为0.825和0.932,Pi与Peso之间的平均差值小于(Peso+Pi)/2中的最小值。结论:机械通气患者镇静时,在线式测定的P0.1与Peso之间以及Pi与Peso之间均具有良好的相关性,Pi较P0.1更适合用于机械通气患者镇静时呼吸努力的无创性评估。Objective:To investigate the correlation and agreement between esophageal pressure(Pc- so) and mean inspiratory pressure(Pi). Methods:12 patients mechanically ventilated were enrolled in the study. Peso was monitored through insertion of esophageal balloon. Infusion of propofol for calm was targetedly controlled and then the airway occlusion pressur (PO. 1),inspiratory time and Pi were measured and counted. Correlation analysis were made between P0. 1 and Peso as well as between Pi and Peso. Results: The correlation coefficient between P0. 1 and Peso was 0. 825, whereas for Pi O. 932. In the agreement analysis, mean difference between Pi and Peso was lower than the minimum of (Peso+Pi)/2. Conclusion: In patients mechanically ventilated, there is good correlation between PO. 1 and Peso as well as between Pi and Peso. Pi is more preferable than P0. 1 in noninvasive assessment of respiratory effort in sedation of patients mechanically ventilated.
