选择性颈外动脉分支栓塞术在鼻、口腔大出血中的应用

摘　　要：目的 :探讨鼻腔、口腔严重出血的非手术治疗方法 ,DSA表现及并发症。方法 :采用 Seldinger′s法 ,以明胶海绵为栓塞材料对 9例鼻和口腔严重出血病人施行急诊超选择性颌内动脉和面动脉栓塞术。结果 :5例出血病人完全止血 ,随访 6～ 36个月无复发 ,1例病人栓塞术后出血明显减少 ,1例病人栓塞后 1月再次出血 ,第二次栓塞后随访 6个月未复发。 2例病人栓塞后活动性出血停止 ,继续接受放疗。结论 :超选择性动脉栓塞治疗严重口、鼻出血简捷、安全、效果肯定 ,可在临床推广应用。Objective:To explore the unoperative method,DSA features and possible complication of nasal and oral hemorrhage.Methods:Absorbable gelatin sponge was used in 9 emergency cases of acute nasal and oral hemorrhage to embolize the internal maxillary and facial artery by using seldinger′s method.Results:After embolization,the bleeding was completey stopped and there was not any recurrence for 6 to 36 months in 5 cases,bleeding was obviously reduced in one case,one case had recurrence one month later after embolization and the bleeding was stopped after second ebmolization.Two cases received radiotherapy after successful embolization.Conclusion:This method is safe and effective on treating the severe nasal and oral hemorrhage.
