摘 要:目的探讨对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)急性发作期患者呼出气一氧化氮(fraction of exhaled nitric oxide,FENO)值及其与肺功能等相关因素的关系。方法选取70例AE-COPD住院患者为研究对象,收集一般资料(姓名、性别、年龄、族别、身高、体质量、体质指数),完成既往疾病史、过敏史、吸烟史及哮喘史的调查问卷,采用瑞典耐尔斯(NIOX)一氧化氮测定仪检测FENO记录肺功能检测数值,包括第1秒用力呼气容积占预计值百分比(FEV1%)及第1秒用力呼气容积/用力肺活量(FEV1/FVC%)。结果 (1)70例AECOPD患者平均FENO值为(26.4±16.64)ppb,最小值为7.1ppb,最大值为99.6ppb;(2)不同年龄组FENO值差异有统计学意义(P<0.05),随着年龄的增大,FENO值逐渐降低;(3)不同体质指数组间FENO值无差异(P>0.05),但随着体质指数的增加,FENO值随之降低;(4)不同级别的肺功能患者FENO值差异有统计学意义(P<0.05),随着患者肺功能的逐渐下降,FENO值随之降低;(5)FENO值与年龄(γ=-0.111,P=0.047)、BMI(γ=-0.098,P=0.042)及FEV1/FVC%(γ=-0.002,P=0.044)呈负相关。结论 FENO测定对COPD患者气道炎症的判断有一定意义。Objective To explore fraction of exhaled nitric oxide(FENO) of the acute exacerbating chronic obstructive pulmonary disease patients and the related factors of pulmonary function.Methods Selected the hospitalized 70 AECOPD patients between 48 and 89 years old as the research objects,collected the general information(name,gender,age,nationality,height and body mass index),completed the questionnaire of medical history,allergic history,smoking history and asthma history,made tests with Sweden NIOX nitric oxide tester,then recorded the detected pulmonary fractions including FEV1% and FEV1/FVC%.Results(1) The average FENO fraction of 70 AECOPD patients was(26.4±16.64)ppb,the minimum fraction was 7.1 ppb,and the maximum fraction was 99.6 ppb;(2) The FENO fraction in different age groups had the statistical difference(P0.05).As people was older,the FENO fraction was gradually reduced;(3) The FENO fraction had no statistical difference among different weight index groups(P0.05).As the weight index became bigger,the FENO fraction was gradually reduced;(4) The FENO fraction had the statistical difference among the pulmonary patients in different degrees(P0.05).As the patient′s pulmonary function declined,the FENO fraction was gradually reduced;(5) The FENO fraction was negatively related with age(γ=-0.111,P=0.047),BMI(γ=-0.098,P=0.042) and FEV1/FVC%(γ=-0.002,P=0.044).Conclusion FENO detection plays an important role in the judgment of airway inflammation for COPD patients.
