乳腺癌钙化X线表现与组织中治疗相关指标免疫组织化学表达的关系

摘　　要：目的探讨乳腺癌钙化的X线表现及其与免疫组织化学的关系。方法新疆医科大学附属肿瘤医院放射科行数字化乳腺X线检查,并经手术病理证实乳腺癌的166例患者。术后标本行免疫组织化学染色,分析测定雌激素受体(estrogen receptor,ER)、孕激素受体(progesterone receptor PR)、C-erb-2、Ki-67、多种耐药性(manydrug resistance,MDR)、肺耐药蛋白(lung resistance protein,LRP)表达情况。结果在细小多形性钙化ER阳性占63.5%(73/115),PR阳性占64.8%(68/105),C-erb-2阴性占81.6%(71/112)。不同钙化形态ER阳性表达(χ2=27.43,P=0.000)、PR阳性表达(χ2=25.20,P=0.000)、Cerb-2阴性表达(χ2=18.79,P=0.001)差异有统计学意义。在钙化数目>35个ER阴性占62.7%(32/51),PR阴性占60.7%(37/61);C-erb-2阳性占64.8%(35/54)。钙化数目与ER阴性表达(χ2=6.160,P=0.045)、PR阴性表达(χ2=6.129,P=0.049)、C-erb-2阳性表达(χ2=9.444,P=0.009)差异有统计学意义。结论细小多形性钙化在病理上表现恶性程度低,复发率低,内分泌治疗效果和预后较好。钙化数目>35个在病理上表现恶性程度高,复发率高,内分泌治疗效果和预后差。根据乳腺癌钙化X线钙化形态、钙化数目特征可初步判断免疫组织化学情况,从而指导临床用药和预后。Objective To explore the X-ray signs of breast cancer′s calcification,and its relationship with immunohistochemistry.Methods A retrospective analysis of 166 cases who lined digital breast X-ray,and took the operation of of pathology.Postoperative specimens were tested by immunohistochemical stain and the expressions of ER,PR,C-erb-2,Ki-67,MDR,LRP were determined.Results In the small pleomorphic calcification in ER positive accounted for 63.5%（73/115）,In PR positive accounted for 64.8%（68/105）,in C-erb-2 negative accounted for 81.6%（71/112）.Different calcified form expressed statistically significant differences in ER positive expression（χ2=27.43,P=0.000）,PR positive expression（χ2=25.20,P=0.000）,C-erb-2 negative expression（χ2=18.79,P=0.001）.The number of calcification which was greater than thirty-five in ER negative accounted for 62.7%（32/51）,in PR negative accounted for 60.7%（37/61）,in C-erb-2 positive accounted for 64.8%（35/54）.The number of calcification expressed statistically significant differences in ER negative expression（χ2=6.160,P=0.045）,PR negative expression（χ2=6.129,P=0.049）,C-erb-2 positive expression（χ2=9.444,P=0.009）.Conclusion Small pleomorphic calcification in pathology the malignant degree is low,the recurrence rate is low,endocrine therapy effect and prognosis is good.The number of calcification which is greater than thirty-five in pathology the malignant degree is high,and the recurrence rate is high,endocrine therapy effect and prognosis is poor.According to X-ray features of breast cancer calcification,it could preliminary estimate immunohistochemical situation,and guide clinical medication and prognosis.
