左室功能正常的不稳定型心绞痛患者血清NT-pro-BNP水平的影响因素探讨

摘　　要：目的探讨左室功能正常的不稳定型心绞痛患者血清NT-pro-BNP水平的影响因素。方法选取133例临床诊断不稳定型心绞痛患者,心超证实左室射血分数>50%,入院后收集患者的一般临床资料,并行冠脉造影,任何一支主要血管狭窄>75%定义为冠脉造影阳性。所有患者采用罗氏CARDIAC proBNP+检测仪测定NT-proBNP水平。结果采用多重线性回归分析探讨NT-proBNP水平的影响因素,纳入变量包括患者年龄、性别、高血压、抽烟、糖尿病、心房纤颤、冠脉支架置入(PCI术)史、左室射血分数(LVEF)值、冠脉造影结果等。结果提示,年龄、心房纤颤和冠脉造影阳性和NT-proBNP水平呈显著正相关,其中冠脉造影结果对NT-proBNP水平的影响最大,心房纤颤其次,年龄的影响最小。结论冠状动脉狭窄可导致心功能正常的不稳定型心绞痛患者血NT-proBNP水平显著升高,同时,心房纤颤和年龄也是NT-proBNP水平升高的独立预测因子。Objective To investigate the factors influencing plasma NT - proBNP levels in ustable angina patients with preserved left ventricular function. Methods A total of 133 patients with primary diagnosis of unstable angina were enrolled into this study. The left ventricular ejection fraction of all patients were 〉 50% confirmed by echocardiogram. The clinical data were collected after admission and angiography was perfoimed by experienced cardiologist. Stenosis 〉 75% in any major vessels was defined as angiography positive. NT- proBNP level was determined by CARDIAC proBNP ＋ machine. Results Multiple linear regression was performed to investigate the fac- tors influencing NT -proBNP levels and the included variables were age, gender, hypertension, smoke, diabetes, atrial fibrillation, histo-ry of PCI, left ventricular ejection fraction, and angiography. The resuhs showed that angiography, age and atrial fibrillation were signifi-cantly correlated with NT - proBNP（ the standard coefficient was 0,384, O. 244 and 0. 264, respectively ; P 〈 0.05 ）. Conclusion NT -proBNP level was found to increase with the significant stenosis of coronary artery. Age and atrial fibrillation were also independently cor-related with NT - proBNP level.
